摘  要:目的探讨大黄素(emodin,EMO)对化疗性静脉炎兔静脉穿刺点促炎因子和生长因子的作用及其机制。方法48只雄性新西兰大白兔分为正常对照组(NC组)、模型对照组(MC组)、EMO外敷治疗组(EMO+MC组)、多磺酸粘多糖乳膏贴膏(MP)治疗组(MP+MC组)、核因子(NFκB)的抑制剂二硫代氨基甲酸吡咯烷铵(APDC)静脉注射结合EMO外敷处理MC组(APDC+EMO+MC组),以及APDC静脉注射治疗MC组(APDC+MC)。造模48 h后收集血清以及耳组织,HE染色评估耳组织的病理变化;ELISA检测TNF-α和IL-6;RTqPCR检测VEGF和bFGF的表达;Western blot检测p-p65、p-IκBα、p-IKKβ蛋白的表达。结果与MC组比,EMO+MC组耳组织炎性浸润和损伤改善,EMO+MC组和APDC+MC组的穿刺点血清TNF-α、IL-6、VEGF、bFGF及耳组织的p-p65、p-IκBα、p-IKKβ的水平均降低,差异均有统计学意义(均P<0.05)。与APDC+MC组比,APDC+EMO+MC组穿刺点血清TNF-α、IL-6、VEGF、bFGF的水平降低,且耳组织的p-p65、p-IκBα、p-IKKβ的表达水平降低,差异均有统计学意义(均P<0.05)。结论EMO通过抑制NF-κB信号通路降低化疗性静脉炎兔静脉穿刺点VEGF、bFGF、TNF-α、IL-6的表达。Objective This study aimed to investigate the effects and mechanisms of emodin(EMO)on the expression of proinflammatory and growth factors at the venous puncture site in chemotherapy-induced phlebitis.Methods Forty-eight male New Zealand white rabbits were divided into six groups:normal control group(NC),model control group(MC),EMO topical treatment group(EMO+MC),polysaccharide mucopolysaccharide lotion plaster treatment group(MP+MC),EMO topical treatment combined with intravenous injection of the NF-κB inhibitor ammonium pyrrolidine dithiocarbamate(APDC+EMO+MC),and intravenous injection of APDC treatment group(APDC+MC).After 48 hours of modeling,serum and ear tissues were collected.Ear tissue pathological changes were evaluated by hematoxylin and eosin staining,and the expression levels of TNF-α,IL-6,VEGF,and bFGF were detected by ELISA and RT-qPCR.The expression levels of p-p65,p-IκBα,and p-IKKβ proteins were detected by Western blot.Results Compared with the MC group,the EMO+MC group showed improved inflammatory infiltration and tissue damage in the ear.The levels of TNF-α,IL-6,VEGF,bFGF,p-p65,p-IκBα,and p-IKKβ in the puncture site serum and ear tissue were all significantly decreased in the EMO+MC and APDC+MC groups(all P<0.05).Compared with the APDC+MC group,the APDC+EMO+MC group showed a significant decrease in the levels of TNF-α,IL-6,VEGF,bFGF,and the expression levels of p-p65,p-IκBα,and p-IKKβ in ear tissue(all P<0.05).Conclusions Emodin can regulate the expression of VEGF,bFGF,TNF-α,and IL-6 at the venous puncture site in chemotherapy-induced phlebitis by inhibiting the NF-κB signaling pathway.
