摘  要:目的观察补肾化瘀方对脱氢表雄酮诱导的多囊卵巢综合征(polycystic ovary syndrome,PCOS)大鼠模型的影响,探讨其作用机制。方法通过颈背部皮下注射脱氢表雄酮(dehydroepiandrosterone,DEHA)溶液构建PCOS大鼠模型,将成模大鼠随机分为中药组、阳性对照组、模型对照组,每组10只,另取10只正常大鼠作为空白对照组。造模成功后分别予以药物干预60天。采用苏木精—伊红(hematoxylin-eosin,HE)染色法观察卵巢形态学变化;用酶联免疫吸附测定(enzyme linked immunosorbent assay,ELISA)检测卵泡刺激素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)、孕酮(progesterone,P)、雌激素(estradiol,E2)总睾酮(testosterone,T)等;实时荧光定量聚合酶链式反应(Real-time,PCR)和蛋白免疫印迹法(Western Blot)分别检测卵巢组织抗缪勒管激素(anti-Müllerian hormone,AMH)、AMH受体2(AMH receptor,AMHR2)mRNA和蛋白表达。结果与空白对照组比较,模型对照组大鼠卵巢组织中可见囊状扩张卵泡增多,极少成熟卵泡,血清LH、T明显升高(P<0.05),AMH及其mRNA水平显著升高(P<0.05),卵巢组织中AMH、AMHR2蛋白表达水平均升高(P<0.05);与模型对照组比较,中药组大鼠卵巢组织中可见成熟卵泡,囊状扩张卵泡及闭锁卵泡减少,血清FSH、P、E2水平显著升高(P<0.05),血清LH、T水平显著降低(P<0.05),AMH及其mRNA的表达水平显著降低(P<0.05),大鼠卵巢组织中AMH、AMHR2的蛋白表达水平明显降低(P<0.05)。结论补肾化瘀方能有效降低T水平,改善PCOS内分泌水平,对AMH及其表达有调节性作用,恢复卵巢功能和形态、促进卵泡发育和成熟。Objective To observe the effect of BuShenHuaYu Formula(BSHYF)resolving blood stasis formula on dehydroepiandrosterone-induced polycystic ovary syndrome(PCOS)rat model and to explore its mechanism of action.Methods The PCOS rat model was constructed by subcutaneous injection of dehydroepiandrosterone solution into the back of the neck,and the rats were randomly divided into Chinese medicine group,positive control group and model control group,10 rats in each group,and 10 normal rats were taken as blank control group.The morphological changes of ovaries were observed by HE staining,follicle stimulating hormone(FSH),luteinizing hormone(LH),progesterone(P)and total testosterone(T)were measured by ELISA.The levels of AMH and AMH receptor 2 mRNA and protein expression were detected by PCR and Western blot,respectively.Results Compared with the blank control group,the ovarian tissues of rats in the model control group showed more cystic dilated follicles and very few mature follicles,significantly higher serum LH and T(P<0.05),significantly higher AMH and its mRNA levels(P<0.05),and higher AMH and AMHR2 protein expression levels in ovarian tissues(P<0.05).In comparison with the model control group,mature follicles,cystic dilated follicles and atretic follicles were reduced,the levels of FSH,P and E2 in serum were significantly increased(P<0.05),the levels of LH and T in serum were significantly decreased(P<0.05),the expression levels of AMH and its mRNA were significantly decreased(P<0.05),and the protein expression levels of AMH and AMHR2 in the ovarian tissues were significantly decreased(P<0.05).Conclusion The formula of tonifying the kidney and resolving blood stasis can effectively reduce the T level,improve the endocrine level of PCOS,have a regulatory effect on AMH and its expression,restore ovarian function and morphology,and promote follicle development and maturation.
