梅毒患者外周血滤泡辅助性T细胞水平及临床意义

摘　　要：目的了解梅毒患者外周血不同表型滤泡辅助性T细胞(Tfh)水平,讨论其临床意义。方法纳入84例梅毒患者,根据病程分为治疗前组、治疗中组、治愈组、血清固定组,以37例健康体检人员作为对照组。采用流式细胞术检测各组外周血CD4^(+)CXCR5^(+)ICOS^(+)、CD4^(+)CXCR5^(+)、CD4^(+)ICOS^(+)Tfh细胞比例,分析Tfh细胞在各组间的差异以及与血清TRUST滴度的相关性。结果与对照组相比,治疗前患者CD4^(+)CXCR5^(+)ICOS^(+)Tfh细胞占外周血CD4^(+)T细胞比例显著上升(P<0.001)。与治疗前相比,CD4^(+)CXCR5^(+)ICOS^(+)Tfh细胞占CD4^(+)T细胞比例在治疗中组(P=0.039)、治愈组(P<0.001)、血清固定组(P=0.009)患者外周血中均显著下降。CD4^(+)CXCR5^(+)ICOS^(+)Tfh细胞占CD4^(+)T细胞比例与血清TRUST滴度呈正相关(r=0.30,P<0.01);CD4^(+)CXCR5^(+)(r=-0.06,P=0.519)、CD4^(+)ICOS^(+)Tfh细胞(r=0.14,P=0.118)占CD4^(+)T细胞比例与TRUST滴度无明显相关。结论梅毒患者外周血中CD4^(+)CXCR5^(+)ICOS^(+)Tfh细胞可能参与梅毒的发生与发展。Objective To investigate the levels of different follicular helper T cells in peripheral blood of syphilis patients and discuss its clinical significance.Methods Eighty-four syphilis patients and 37 healthy controls were enrolled from Dermatology Hospital of Southern Medical University from May 2015 to March 2016.Flow cytometry was used to measure CD4^(+)CXCR5^(+)ICOS^(+)、CD4^(+)CXCR5^(+)、CD4^(+)ICOS^(+)Tfh cells in the peripheral blood.Correlation of TRUST titers with different Tfh cells in syphilis patients,including untreated,treated,cured or sero-fast,was analyzed.Results The proportion of CD4^(+)CXCR5^(+)ICOS^(+)Tfh cells in CD4^(+)T cells was significantly higher in the pre-treatment group than in healthy controls(P<0.001).The proportions of CD4^(+)CXCR5^(+)ICOS^(+)Tfh cells in CD4^(+)T cells in the post-treatment,cured and sero-fast syphilis groups were significantly lower than that in the pre-treatment group(P=0.039,P<0.001,P=0.009).The proportion of CD4^(+)CXCR5^(+)ICOS^(+)Tfh cells in CD4^(+)T cells was positively correlated with TRUST titers(r=0.30,P<0.01).However,the proportion of neither CD4^(+)CXCR5^(+)Tfh cells nor CD4^(+)ICOS^(+)Tfh cells in CD4^(+)T cells was correlated with TRUST titers(P=0.519,P=0.118).Conclusion Tfh cells in peripheral blood may be involved in the onset and the development of syphilis.
