产前超声心动图诊断左无名静脉走行异常的临床意义

摘　　要：目的:探讨左无名静脉(Left innominate vein,LINV)走行异常产前超声心动图诊断的临床意义。方法:对33例LINV走行异常胎儿产前超声心动图三血管观及三血管气管观LINV走行及与周边组织的关系进行观察,总结分析LINV走行异常的产前超声心动图特征。结果:本研究33例胎儿LINV均未走行于胸腺后方、主动脉弓上方,其中18例胎儿走行于主动脉弓下,合并法洛四联症3例,合并右位主动脉弓1例,合并室间隔缺损1例;12例胎儿走行于胸腺内;2例胎儿走行于胸腺前;1例胎儿走行于气管后。结论:LINV走行异常在三血管观及三血管气管观具有特征声像图表现,产前超声心动图诊断LINV走行异常对生后心脏介入手术或心外手术方式的选择有临床意义。Objective:To investigate the clinical significance of fetal echocardiography in the prenatal diagnosis of abnormal course of left innominate vein(LINV).Methods:Thirty-three fetuses with abnormal course of LINV were retrospectively analyzed.The relationship between the LINV and the surrounding tissue was observed in the three-vessel view and the three-vessel and tracheal view,in order to summarize and analyze the fetal echocardiographic characteristics of abnormal course of LINV.Results:Thirty-three cases of abnormal course of LINV when the innominate vein did not run behind the thymus gland or above the aortic arch were diagnosed by fetal echocardiography in this study.Among them,18 cases ran below the aortic arch including 3 with Tetralogy of Fallot,1 with right aortic arch,1 with ventricular septal defect;12 cases ran inside the thymus gland;2 cases ran in front of the thymus gland;and 1 case ran behind the trachea.Conclusion:The abnormal course of LINV has characteristic sonograms in three-vessel view and three-vessel tracheal view.Prenatal echocardiography diagnosis of abnormal course of LINV has clinical significance for the selection of cardiac intervention or surgery.
