药物禁忌证规则在合理用药监测系统中的精细化设置

摘　　要：目的构建禁忌证的精细化审核模式,为进一步提高处方审核效率、规范合理用药寻找改进方向和措施。方法收集我院合理用药监测系统2020年8月—2021年5月门诊所有禁忌证监测数据,进行回顾性分析,并根据临床实际诊疗情况和循证证据,探索禁忌证的精细化审核规则。结果2020年8月—2021年5月有关禁忌证提示的一共3078条,其中黑灯提示913条,占29.66%;红灯提示2165条,占70.34%。根据我院实际情况精细化维护禁忌证规则15432条。规则精细化设置前后禁忌证问题占比分别为14.8%和11.4%(P<0.05)。结论对禁忌证规则进行精细化设置促进了处方前置审核工作的有效开展,充分体现了药师在合理用药方面的积极作用,展现出药师的专业性。Objective To build a refined review mode for contraindications,and to further improve the efficiency of prescription review and standardization of rational drug use.Methods The monitoring data of all contraindications in the out-patient clinic of the prescription automatic screening system of our hospital from August 2020 to May 2021 were analyzed retrospectively.According to the actual clinical diagnosis and evidence-based evidence,the refined review rules of contraindications were determined.Results From August 2020 to May 2021,there were 3078 tips on contraindications,including 913(29.66%)black lights tips and 2165(70.34%)red light tips.According to the actual situation of our hospital,15432 rules on contraindications should be carefully maintained.The proportion of contraindications before and after the refined rule setting was 14.8%and 11.4%(P<0.05)with significant difference.Conclusion The refined setting of contraindication rules effectively promote the development of prescription preview and fully reflect the positive role of pharmacists in rational drug use.
