醒脑静注射液联合精油穴位按摩治疗缺血性脑卒中后脑血流动力学及神经损伤的影响

摘　　要：目的:探讨醒脑静注射液联合复方精油穴位按摩对缺血性脑卒中后脑血流动力学及神经损伤的影响。方法:随机选取90例缺血性脑卒中患者,平均分为3组:联合组、醒脑静注射液组、常规治疗组,每组30例。常规治疗组给予常规治疗,醒脑静注射液组给予常规治疗+醒脑静注射液静脉注射,联合组给予常规治疗+精油穴位按摩+静脉输注醒脑静注射液,疗程1个月。评估三组患者治疗前(T_(0))、治疗3 d(T_(1))、7 d(T_(2))、14 d(T_(3))、30 d(T_(4))的收缩期峰值流速(Peak Systolic Velocity,PSV)、血管特性阻抗(Vascular Characteristic Impedance,VCI)、动态阻力(Dynamic Resistance,DR)等脑血流参数、血清神经功能相关指标及炎性因子变化。同时用美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)及Barthel指数(Barthel Index,BI)评价患者的恢复情况。结果:与T_(0)相比,醒脑静注射液组和联合组患者T_(2)、T_(3)、T_(4)时PSV明显增加(P<0.05),且血清星形胶质源性蛋白(Serum astroglia-derived protein,S100β)、神经元特异性烯醇化酶(Neuron-Specific Enolase,NSE)、VCI和DR含量均明显降低(P<0.05)。与联合组患者相比,醒脑静注射液组和常规治疗组患者T_(3)、T_(4)时BI和PSV明显降低(P<0.05),而NIHSS评分、BI值及炎性因子水平均明显升高(P<0.05)。结论:醒脑静注射液联合复方精油穴位按摩可明显改善缺血性脑卒中患者的脑血流动力学指标,有明显脑保护作用。Objective:To investigate efficacy of the Xingnaojing injection(醒脑静注射液)plus acupoint massage with essential oil on cerebral hemodynamics and neurological damage in patients with cerebral ischemic stroke(CIS).Methods:A total of 90 patients with CIS were randomly selected,evenly divided into 3 groups:the combined group,the Xingnaojing injection group,the normol group,with 30 patients in each group.The normol group was given routine treatment,the Xingnaojing injection group was given routine treatment plus the Xingnaojing injection,and the combined group was given routine treatment plus essential oil acupoint massage and the Xingnaojing injection,for 1 month.The cerebral blood flow parameters,such as peak systolic velocity(PSV),vascular characteristic impedance(VCI),dynamic resistance(DR)before treatment(T_(0)),3 d(T_(1)),7 d(T_(2)),14 d(T_(3)),30 d(T_(4))in the three groups were evaluated.The national institutes of health stroke scale(NIHSS)and Barthel index(BI)were used to evaluate recovery.Results:Compared with T_(0),PSV in the Xingnaojing injection group and combined group on T_(2)to T_(4)was increased(P<0.05),and serum astroglia-derived protein(S100β),neuron-specific enolase(NSE),VCI and DR were significantly decreased(P<0.05).Compared with the combined group,BI and PSV were decreased significantly on T_(3)to T_(4)(P<0.05),and NIHSS,BI index and inflammatory factors were all significantly higher(P<0.05).Conclusion:The Xingnaojing injection plus acupoint massage with essential oil can obviously ameliorate cerebral hemodynamics and protect patients with cerebral ischemic stroke.
