内外同治法治疗神经根型颈椎病47例

摘　　要：目的:观察内外同治法治疗神经根型颈椎病的临床疗效,分析其作用机制。方法:募集神经根型颈椎病患者47例,随机分组,分别给予内外同治法和单纯推拿手法治疗,记录患者治疗前后疼痛数字评价量表(Numerical Rating Scales,NRS)评分、颈椎功能障碍指数(Neek Disability Index,NDI)评分、颈部肌肉定量评估(颈部斜方肌、胸锁乳突肌硬度、张力测定),并对治疗后两组的疗效进行统计分析。结果:治疗后,治疗组临床有效率明显高于对照组(P<0.05),经治疗两组患者的疼痛NRS评分、NDI评分、斜方肌与胸锁乳突肌张力和刚度均显著下降(P<0.05),治疗组患者的斜方肌、胸锁乳突肌平均张力、刚度降低更为明显(P<0.05)。结论:用内外同治法治疗神经根型颈椎病可以改变患者肩颈部肌肉力学性能,疗效显著。Objective:To observe the clinical effect of internal and external treatment on cervical spondylotic radiculopathy and analyze its mechanism.Methods:A total of 47 patients with cervical spondylotic radiculopathy were randomly divided into groups and treated by internal and external treatment and manipulation respectively.The pain digital score,cervical dysfunction index score and quantitative evaluation of cervical muscles(hardness and tension of cervical trapezius muscle and sternocleidomastoid muscle)before and after treatment were recorded and statistically analyzed.The curative effects in the two groups were statistically analyzed after treatment.Results:After treatment,the clinical effective rate in the treatment group was significantly higher than the control group(P<0.05).After treatment,the pain digital score,cervical dysfunction index score,tension and stiffness of trapezius muscle and sternocleidomastoid muscle in the treatment group were decreased significantly(P<0.05),and the average tension and stiffness of trapezius muscle and sternocleidomastoid muscle in the treatment group were decreased more significantly(P<0.05).Conclusion:Internal and external treatment can change the mechanical properties of shoulder and neck muscles in patients with cervical spondylotic radiculopathy,and has a significant effect in the treatment.
