针刺治疗眶壁骨折术后迟发性面瘫案

摘　　要：面瘫以口、眼向一侧歪斜为主,表现为突然出现一侧面部肌肉板滞、麻木、瘫痪,口角下垂歪向健侧,患侧不能皱眉、闭目、鼓气等。在临床中多使用神经营养药物、糖皮质激素或针灸理疗等方式进行治疗,然而激素治疗有不良反应,手术费用高且过程存在一定风险,故寻求一种更加安全有效的治疗方案显得尤为重要。目前探究针刺对眶壁骨折术后迟发性面瘫的治疗效果,以求寻找更加安全有效的治疗方案。患者接受针刺治疗后,其面部功能恢复正常,可见针刺治疗迟发性面瘫疗效显著。文章通过对于眶壁骨折术后迟发性面瘫可能存在的发病机制以及针刺治疗作用原理进行讨论,发现该病可能与面神经损伤、微循环障碍和神经营养因子有关,根据“证同则治同”的原则,选用针刺疗法,辨证治疗,恰当选穴,有较好的治疗效果。本次研究旨在为该病的临床治疗和研究提供新的思路。Facial paralysis is mainly tilted by the mouth and eyes to one side,manifested as the sudden appearance of one side of the facial muscle plate,numbness,paralysis,the corners of the mouth droop to the healthy side,the affected side cannot frown,close the eyes,puff up,etc..In clinical practice,neurotrophic drugs,glucocorticoids or acupuncture physiotherapy are used for treatment,but hormone therapy has adverse reactions,high surgical costs and certain risks in the process,so it is particularly important to seek a safer and more effective treatment plan.In this paper,the therapeutic effect of acupuncture on delayed facial paralysis after orbital wall fracture operation was explore in order to find a more safe and effective treatment scheme.After receiving acupuncture treatment,the facial function of the patients recovered to normal.It can be seen that acupuncture has a significant effect on delayed facial paralysis.The possible pathogenesis of delayed facial paralysis after orbital wall fracture operation and the application principle of acupuncture are discussed as follow,it was found that the disease may be related to facial nerve injury,microcirculation disorders and neurotrophic factors,and according to the principle of“evidence is the same,the same treatment is the same”,acupuncture therapy,differential treatment,appropriate acupoint selection,and good treatment effect are selected.This study provides a new idea for clinical treatment and research of the disease.
