摘  要:目的检测miRNA505-3P在系统性红斑狼疮(SLE)患者血浆中的表达水平,探讨其在SLE发病机制中的作用及临床意义。方法收集2020年9月至2022年5月在武汉市汉口医院风湿科初次就诊并确诊为SLE的患者174例作为SLE组,根据系统性红斑狼疮活动指数(SLEDAI)评分将其分为活动组(92例)和非活动组(82例)。随机选取同期在该院体检的健康体检者60例作为健康对照组。采用实时荧光定量PCR检测所有受试者血浆中miRNA505-3P表达水平,采用酶联免疫吸附试验检测血浆中高迁移率族蛋白1(HMGB1)水平,采用Pearson相关分析miRNA505-3P表达水平与SLEDAI评分和HMGB1水平的关系;绘制受试者工作特征曲线分析miRNA505-3P表达水平对SLE的诊断价值。结果与健康对照组相比,SLE组血浆中miRNA505-3P和HMGB1水平显著增加(P<0.05),且二者在活动组中的水平均高于非活动组(P<0.05)。Pearson相关分析结果显示,miRNA505-3P与SLEDAI评分和HMGB1呈正相关(r=0.7344、0.6475,P<0.05)。SLE患者血浆中miRNA505-3P诊断SLE曲线下面积为0.9699(95%CI:0.9500~0.9898),当miRNA505-3P最佳截断值为0.7650时,诊断SLE的灵敏度为96.67%,特异度为83.33%。以miRNA505-3P的最佳截断值(0.7650)为分界值,将SLE患者分为高表达组和低表达组,两组患者年龄、性别和是否累及肾脏情况比较,差异均无统计学意义(P>0.05),但ANA阳性、抗双链DNA抗体阳性比例和SLEDAI评分比较,差异均有统计学意义(P<0.05)。结论miRNA505-3P表达水平在SLE患者中显著升高,其高表达与SLEDAI评分呈正相关。miRNA505-3P可作为辅助诊断SLE,尤其是活动性SLE的标志物,且可能通过调控HMGB1的表达促进SLE的发生、发展。Objective To detect the expression levels of miRNA505-3P in plasma of patients with systemic lupus erythematosus(SLE)and explore its role in the pathogenesis of SLE and clinical significance.Methods Totally 174 patients,who firstly diagnosed with SLE in Rheumatology Department in Wuhan Ha-nkou hospital from September 2020 to May 2022,were collected and divided into the active group(92 cases)and the inactive group(82 cases)according to the SLE activity index(SLEDAI)score.At the same time,60 healthy people,who took physical examination in the hospital,were randomly selected as the healthy control group.The expression level of miRNA505-3P in plasma of all subjects was detected by real-time fluorescence quantitative PCR.The plasma high mobility group protein 1(HMGB1)levels were detected by enzyme-linked immunosorbent assay.Pearson correlation analysis was adopted to analyze the correlation between miR-NA505-3P and SLEDAI score and HMGB1.The receiver operating characteristic was plotted to analyze the di-agnostic value of miRNA505-3P on SLE.Results Compared with the healthy control group,the expression levels of plasma miRNA505-3P and HMGB1 increased significantly in the SLE group(P<0.05),and the ex-pression levels of plasma miRNA505-3P and HMGB1 in the active group were higher than those in the inac-tive group(P<0.05).Pearson correlation analysis showed that miRNA505-3P was positively correlated with SLEDAI and HMGB1(r=0.7344,0.6475,P<0.05).The area under the curve of miRNA505-3P was 0.9699(95%CI:0.9500-0.9898).When the optimal cut-off value of miRNA505-3P was 0.7650,the sen-sitivity was 96.67%,the specificity was 83.33%.According to the optimal cut-off value of miRNA505-3P,the SLE patients were divided into high expression group and low expression group,and the difference of age,gen-der and whether the kidneys were involved in or not between the two groups had no statistical significance(P<0.05),but the difference of proportions of positive ANA and double-stranded DNA antibody and SLE-DAI score between the two group
