基于倾向评分匹配回顾性评价人参-山茱萸治疗慢性心力衰竭的临床疗效

摘　　要：目的:基于倾向评分匹配(Propensity Score Matching,PSM)探讨参萸煎治疗慢性心力衰竭的临床疗效。方法:选取2020年1-12月于辽宁中医药大学附属第二医院住院的84例慢性心力衰竭医案,将其分为两组。将43例就诊期间接受人参-山茱萸治疗医案的患者纳入观察组,41例未接受人参-山茱萸治疗医案的患者纳入对照组。运用PSM最邻近匹配法进行1∶1匹配后有38组匹配成功,且基线资料均衡。治疗2周后,比较生化指标[N末端B型利钠肽原(N-terminal Probrain Natriuretic Peptide,NT-proBNP)、肌钙蛋白T(Troponin T,cTnT)]和心功能指标[左心室射血分数(Left Ventricular Ejection Fraction,LVEF)、每搏心输出量(Stroke Volume,SV)、心功能分级]的变化,予临床疗效评价。结果:观察组总有效率高于对照组(P<0.05);治疗后两组的生化指标、心功能指标均改善(P<0.05),观察组更显著(P<0.05)。结论:参萸煎治疗慢性心力衰竭疗效显著,可降低血清中NT-proBNP、cTnT水平,增加LVEF、SV水平,改善心功能,缓解临床症状。Objective:To explore the clinical efficacy of the Shenyu decoction(参萸煎)on chronic heart failure(CHF)based on propensity score matching(PSM).Methods:The 84 cases of chronic heart failure hospitalized in the Second Affiliated Hospital of Liaoning Chinese Medicine University from January to December 2020 were selected and divided into two groups.Forty-three patients who received Renshen(Panax ginseng)-Shanzhuyu(Corni fructus)during the treatment were divided into the observation group,and 41 patients who did not receive Renshen-Shanzhuyu were divided into the control group.After using the PSM nearest neighbor matching method for 1∶1 matching,38 sets of matching were successful,and the basic data was balanced.After 2 weeks of treatment,biochemical indexes[N-terminal pro-B-type natriuretic peptide(NT-proBNP),troponin T(cTnT)]and cardiac function indexes[left ventricular ejection fraction(LVEF),cardiac output per stroke(SV),cardiac function classification],and the clinical efficacy were compared.Results:The total effective rate in the observation group was higher than the control group(P<0.05);the biochemical indexes and cardiac function indexes in the two groups were ameliorated after treatment(P<0.05),and those in the observation group were more significant(P<0.05).Conclusion:The Shenyu decoction is effective on chronic heart failure.It can reduce serum NT-proBNP and cTnT levels,increase LVEF and SV levels,improve heart function,and relieve clinical symptoms.
