摘 要:目的探讨多重实时荧光PCR法联检13种呼吸道病原体在呼吸道感染早期诊断中的价值。方法收集天水市第一人民医院2021年8月—2022年4月共312例呼吸道疾病住院患者作为研究对象,同时采集咽拭子及深部痰液样本,均接受呼吸道病原体核酸检测,分析上呼吸道疾病及下呼吸道疾病病原体核酸检测结果。结果在312份呼吸道样本中有274份检测到病原体,检出率为87.82%,其中有59份(阳性占比21.53%)样本中检测到两种及以上病原体。上呼吸道疾病共42例,检测到病原体33例,检出率为78.60%,病原体核酸检出率前三位为乙型流感病毒、呼吸道合胞病毒、人鼻病毒。下呼吸道疾病共270例,检测到病原体241例,检出率为89.30%,病原体核酸检出率前三位为肺炎链球菌、呼吸道合胞病毒、流感嗜血杆菌。结论通过多重实时荧光PCR法对引起呼吸道疾病的常见病原体进行核酸检测,能辅助临床快速定位病原体,确定感染类型,为呼吸道感染疾病的早期诊断提供实验室依据。Objective To evaluate the value of multiple real-time PCR for detection of 13 respiratory pathogens in early diagnosis of respiratory tract infection.Methods A total of 312 inpatients with respiratory diseases were collected from August 2021 to April 2022 in our hospital as the research subjects,and deep sputum samples and throat swab samples were collected,and nucleic acid test results of pathogens of upper and lower respiratory diseases were analyzed.Results Pathogens were detected in 274 of 312 respiratory tract samples,with a detection rate of 87.82%,and two or more pathogens were detected in 59 samples(positive ratio 21.53%).There were 42 cases of upper respiratory diseases,33 cases of pathogens were detected,and the detection rate was 78.60%.The top three pathogens in nucleic acid detection rate were influenza B virus,respiratory syncytial virus and human rhinovirus.There were 270 cases of lower respiratory tract diseases,and 241 cases of pathogens were detected,with a detection rate of 89.30%.The top three pathogens in nucleic acid detection rate were Streptococcus pneumoniae,respiratory syncytial virus and Haemophilus influenzae.Conclusion Nucleic acid detection of common pathogens causing respiratory tract diseases by multiplex real-time fluorescence PCR can assist the clinical diagnosis of respiratory tract infectious diseases to quickly locate pathogens,determine the type of infection,and provide laboratory basis for the early diagnosis of respiratory tract infectious diseases.
