摘 要:目的分析某院2019—2021年铜绿假单胞菌(PA)临床分布及耐药性,为抗菌药物的合理应用提供依据。方法回顾性分析2019年1月至2021年12月北京丰台医院检出的312株PA,包括标本分布、科室分布、耐药性。结果PA主要来自痰/肺泡灌洗液(69.9%)、尿液(14.1%)、分泌物(9.6%)。PA分离前3的科室为重症监护室(28.5%)、呼吸内科(24.4%)、神经内科(9.3%)。PA对阿米卡星的总耐药率最低(4.5%),其次为妥布霉素(6.4%)、庆大霉素(9.6%);PA对氨曲南的总耐药率最高(25.0%),其次为亚胺培南(23.7%)、美罗培南(19.9%)。312株PA中,耐碳青霉烯类铜绿假单胞菌(CRPA)74株,占23.7%。结论PA科室分布广泛、耐药现象严重,临床医生要根据药敏试验结果规范应用抗菌药物,减少耐药菌的产生。Objective To analyze the clinical distribution and drug resistance of Pseudomonas aeruginosa(PA)in a hospital from 2019 to 2021,in order to provide a basis for the rational application of antibiotics.Methods A retrospective analysis was conducted on 312 strains of PA detected at Beijing Fengtai Hospital from January 2019 to December 2021,including sample distribution,department distribution and drug resistance.Results PA was mainly from sputum/bronchoalveolar lavage fluid(69.9%),urine(14.1%)and secretion(9.6%).The top three departments of PA isolation were intensive care unit(28.5%),respiratory medicine department(24.4%)and neurological department(9.3%).The total drug resistance rate of PA to amikacin was the lowest(4.5%),followed by tobramycin(6.4%)and gentamicin(9.6%).The total drug resistance rate of PA to aztreonam was the highest(25.0%),followed by imipenem(23.7%)and meropenem(19.9%).Among the 312 strains of PA,74 strains were carbapenem-resistant Pseudomonas aeruginosa(CRPA),accounting for 23.7%.Conclusion PA departments are widely distributed and drug resistance is serious.Clinicians should standardize the application of antibiotics according to the results of drug sensitivity test to reduce the generation of drug-resistant bacteria.
