快速康复外科理念结合中医特色护理在股骨粗隆间骨折PFNA内固定患者中的应用效果

摘　　要：目的分析快速康复外科(FTS)理念结合中医特色护理在股骨粗隆间骨折防旋型股骨近端髓内钉(PFNA)内固定患者中的应用效果。方法按数字分配法将本院80例股骨粗隆间骨折PFNA内固定患者分为对照组和观察组,各40例。对照组实施西医常规护理,观察组实施FTS理念结合中医特色护理。比较两组的干预效果。结果术后30 d,观察组的骨钙素(BGP)、骨保护素(OPG)、Ⅰ型前胶原氨基端延长肽(PⅠNP)水平及及健康调查简表(SF-36)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分优于对照组(P<0.05)。观察组的并发症总发生率低于对照组,护理满意度高于对照组(P<0.05)。观察组的手术时间、术后首次排气时间与下床活动时间、住院时长均短于对照组(P<0.05)。结论FTS理念结合中医特色护理能够改善股骨粗隆间骨折PFNA内固定患者骨代谢指标,促进术后恢复,缓解患者不良情绪,减少并发症,提高护理满意度及生活质量。Objective To analyze the application effect of fast track surgery(FTS)concept combined with traditional Chinese medicine characteristic nursing in patients with proximal femoral nail antirotation(PFNA)internal fixation of intertrochanteric fracture.Methods According to the number allocation method,80 patients with PFNA internal fixation of intertrochanteric fracture were divided into control group and observation group,with 40 cases in each group.The control group received routine western medicine nursing,and the observation group received FTS concept combined with traditional Chinese medicine characteristic nursing.The intervention effects of the two groups were compared.Results At 30 d after operation,the levels of bone gla protein(BGP),osteoprotegerin(OPG)and procollagen typeⅠamino-terminal propeptide(PⅠNP)and the scores of 36-Item Short-Form Health Survey(SF-36),Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)were better than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the nursing satisfaction was higher than that in the control group(P<0.05).The operation time,postoperative first exhaust time and ambulation time and hospital stay of the observation group were shorter than those of the control group(P<0.05).Conclusion FTS concept combined with traditional Chinese medicine characteristic nursing can improve the bone metabolism indexes of patients with PFNA internal fixation of intertrochanteric fracture,promote postoperative recovery,relieve patients'negative mood,reduce complications,and improve nursing satisfaction and quality of life.
