持续性心房颤动射频消融术前血清醛固酮与左心房基质相关性研究

摘　　要：目的探究持续性心房颤动(房颤)射频消融术前血清醛固酮及术中左心房低电压区之间的相关性。方法入组2021年3月至2022年3月就诊于阜阳市人民医院心内科的持续性房颤拟行射频消融手术的患者60例,收集术前血清醛固酮水平及术中低电压区等数据,将单因素分析中差异有统计学意义的变量进行多因素Logistic回归分析,分析影响左房低电压区的独立危险因素,采用线性回归分析血清醛固酮水平与左房低电压区面积的相关性。结果多因素Logistic回归分析表明,血清醛固酮水平(OR=1.031,95%CI:1.012~1.051)是持续性房颤患者左房低电压区的独立危险因素。ROC曲线显示,醛固酮水平预测左房低电压区的曲线下面积为0.811(95%CI:0.689~0.982,P<0.01),预测血清醛固酮是否有左房低电压区的最佳截断值为157 ng/dl,敏感度76.5%,特异度96.3%。线性回归分析显示血清醛固酮水平越高,左房低电压区面积越大(标准化系数0.777,P<0.01)。结论在持续性房颤患者中,血清醛固酮水平是左房存在低电压区的独立危险因素,血清醛固酮水平越高,左房低电压区面积越大。Objective To discuss the correlation between preoperative level of serum aldosterone(ALD)and intraoperative left atrial low voltage zone(LVZ)in patients with persistent atrial fibrillation(PeAF)before radiofrequency ablation(RFA).Methods PeAF patients(n=60)received planed RFA were chosen Department of Cardiovascular Medicine in Fuyang Hospital from Mar.2021 to Mar.2022 for collecting the data of preoperative ALD level and intraoperative left atrial LVZ.The variables having differences with statistical significance in single�factor analysis were given a multi-factor Logistic regression analysis.The independent risk factors affecting left atrial LVZ were analyzed.The correlation between serum ALD level and left atrial LVZ was analyzed by using linear regression analysis.Results The results of multi-factor Logistic regression analysis showed that serum ALD level(OR=1.031,95%CI:1.012~1.051)was an independent risk factor of and left atrial LVZ in PeAF patients.The results of ROC curve analysis showed that AUC of ALD level was 0.811(95%CI:0.689~0.982,P<0.01)in predicting left atrial LVZ,and optimal cutoff value was 157 ng/dl,sensitivity was 76.5%and specificity was 96.3%.The results of linear regression analysis showed that the higher the serum ALD level,the larger the left atrial LVZ(standardized coefficient=0.777,P<0.01).Conclusion The level of serum ALD is an independent risk factor of left atrial LVZ in PeAF patients,and the higher the serum ALD level,the larger the left atrial LVZ.
