摘 要:目的 探讨心房颤动患者血浆中磷脂酰肌醇3-激酶(posphoinositide 3-kinase, PI3K)、蛋白激酶B(protein kinase B, Akt)的表达,并分析其与心房颤动发病以及射频消融术后复发的相关性。方法 选取2019年1月~2020年11月蚌埠医学院第一附属医院住院确诊且均首次接受射频消融术的心房颤动患者145例为实验组,此外选取同一时期至蚌埠医学院第一附属医院健康体检的心电图表现为窦性节律的健康体检者120例作为对照组。采用酶联免疫吸附试验(enzyme linked immunosorbent assay, ELISA)检测受试者血浆中PI3K、Akt水平;Western blot法检测外周血单核细胞(peripheral blood mononuclear cells, PBMCs)磷酸化PI3K、Akt蛋白表达水平,对术后患者进行为期11.21±0.42个月的随访,分析它们在心房颤动患者中发病以及射频消融术后复发的表达意义。结果 ELISA检测结果显示,实验组中血浆PI3K、Akt水平显著高于对照组(P<0.01)。Western blot法检测结果显示,实验组磷酸化PI3K、Akt蛋白表达水平较对照组均呈现下调趋势,差异有统计学意义(P<0.01),表明磷酸化PI3K、Akt蛋白表达水平在心房颤动患者中的表达是降低的;与射频消融术后未复发组患者血浆PI3K、Akt比较,复发组患者明显降低(P<0.001)。结论 心房颤动的发生、发展以及射频消融术后的复发可能与PI3K/Akt信号通路表达下调有关。Objective To investigate the expression of posphoinositide 3-kinase(PI 3K)and protein kinase B(Akt)in plasma of patients with atrial fibrillation,and to analyze the correlation between the expression of them and the incidence of atrial fibrillation and the recurrence of radiofrequency ablation.Methods A total of 145 patients with atrial fibrillation who were admitted to the First Affiliated Hospital of Bengbu Medical College from January 2019 to November 2020 and received radiofofency ablation for the first time were selected as the experimental group,120 patients who underwent physical examination at the First Affiliated Hospital of Bengbu Medical College during the same period were selected as the control group.The expression levels of PI 3K and Akt in plasma were detected by enzyme linked immunosorbent assay(ELISA).the protein expression levels of phosphorylation PI 3K and Akt in peripheral blood mononuclear cells(PBMCs)were detected by Western blot.Patients were followed up for(11.21±0.42)months after surgery,to analyze the significance in patients with atrial fibrillation and their recurrence after radiofrequency ablation.Results ELISA results showed that the levels of PI 3K and Akt in plasma in experimental group were significantly higher than those in the control group(P<0.01).Western blot test results showed that the protein expression levels of phosphorylation PI 3Kand Akt in the experimental group were significantly lower than those in the control group,and there were significant differences between the two groups(P<0.01),indicating that the protein expression levels of phosphorylation PI 3K and Akt were decreased in patients with atrial fibrillation.Compared with plasma PI 3K and Akt in patients in the non-recurrent group after radiofrequency ablation,plasma PI 3K and Akt were significantly decreased in the recurrent group,(P<0.001).Conclusion The occurrence,development and recurrence of atrial fibrillation after radiofrequency ablation may be related to the down-regulation expression of PI 3K/Akt sig
