急诊医护团队主导下成人体外心肺复苏患者院内安全转运的研究

摘　　要：目的 探讨急诊医护团队主导下成人体外心肺复苏(ECPR)患者院内安全转运的方法,为成人ECPR患者院内安全转运提供借鉴。方法 选取2020年1月—2022年4月南京医科大学第一附属医院急诊抢救室救治的23例成人ECPR患者作为研究对象。建立急诊医护转运团队,对患者院内安全转运进行全程管理。记录患者院内转运目的地及所用时间,转运突发情况及转运不良事件。比较23例患者转运前、转运中、转运后的心率、血压、血氧饱和度、呼吸频率、血流量、体外膜肺氧合(ECMO)泵转速,ECMO水箱温度的差异。结果 23例成人ECPR患者进行43次院内转运,转运途中遇到4次突发情况,未发生转运不良事件,均安全抵达转运目的地。23例患者转运前、转运中、转运后的心率、血压、血氧饱和度、呼吸频率、血流量、ECMO泵转速,ECMO水箱温度比较,均无统计学意义(P>0.05)。结论 急诊医护团队能够为成人ECPR患者院内安全转运护航,院内安全转运经验可供同行借鉴。Objective To explore the method of safe in-hospital transport of adult patients with extracorporeal cardiopulmonary resuscitation(ECPR)led by emergency medical team,so as to provide reference for safe in-hospital transport of adult patients with ECPR.Methods Twenty-three adult patients with ECPR treated in the emergency room of our hospital from January 2020 to April 2022 were selected as the research objects.The emergency medical and nursing transport team should be established to manage the whole process of patients'safe transport in hospital.The destination and time of in-hospital transport,transport emergencies and adverse events were recorded.The heart rate,blood pressure,oxygen saturation,respiratory rate,blood flow,ECMO pump speed and ECMO tank temperature of 23 patients were compared before,during and after transport.Results Twenty-three adult patients with ECPR underwent 43 times of in-hospital transport,encountered 4 emergencies during the transport,no adverse events occurred,and all arrived at the destination safely.There were no significant differences in heart rate,blood pressure,oxygen saturation,respiratory rate,blood flow,ECMO pump speed and ECMO tank temperature among 23 patients before,during and after transport(P>0.05).Conclusion Emergency medical team can escort the safe transport of adult patients with ECPR in hospital.The experience of safe transport in hospital can be used for reference by peers.
