健脾滋肾法针药合用治疗低言语识别性耳聋的疗效观察

摘　　要：目的比较健脾滋肾法指导下针药合用治疗低言语识别率耳聋患者的临床疗效。方法2017年6月~2019年12月在四川省成都中医药大学附属医院、四川省峨眉山市中医院和四川大学望江校区医院就诊本病患者203例,分为实验组共68例82耳,采用针药合用治疗;对照组1共67例80耳,单用针刺治疗;对照组2共68例80耳,单用中药治疗。分别在治疗第0d、2w、4w和6w4个时间窗对3个组患者行听阈及言语识别率测定,疗程结束后随访半年。比较分析3个治疗时间窗口各组疗效差异。结果在治疗周期内,三组患者的纯音听阈阈值均随治疗时间的延长而提升。在不同治疗阶段,实验组与对照组1及对照组2的治疗结果组间比较差异有统计学意义(P<0.05),言语识别率均随着治疗时间增加而提升,但对照组1与对照组2的组间比较差异无性统计学意义(P>0.05)。结论健脾滋肾法指导下的针刺、中药及针药合用均能改善低言语识别性耳聋患者听阈水平及言语识别率,尤以针药合用组疗效最佳。Objective To compare the therapeutic effect of acupuncture therapy combined with herbal medicine on deafness with low speech recognition scores(LSRS)guided under the treating principle of invigorating the Spleen and nourishing the Kidney.Methods Included in this trial were 203 patients with deafness showing LSRS,collected from the Affiliated Hospital of Chengdu University of TCM,Emeishan TCM Hospital and Wangjiang campus hospital of Sichuan University,during the period from June 2017 to December 2019.They were divided into three groups,with 68 cases(82 ears)in the experimental group(EG)treated by acupuncture combined with herbal medicine,67(80 ears)in the control group 1(CG1)treated by acupuncture simply,and 68(80 ears)in the control group 2(CG2)treated by herbal medicine only.Pure tone hearing threshold level(PTHTL)and LSRS were determined in 4 time windows,i.e.on Day 0,and by the end of 2nd,4rd and 6th weeks during the therapeutic period respectively.Thereafter,all these patients were followed up for half a year.Then,the therapeutic effects were comparatively analyzed by the end of 2nd,4rd and 6th therapeutic week respectively.Result During the therapy course,the cases in 3 groups all showed their PTHTL improved gradually as the treating time advancing forward.Moreover,the therapeutic effect of cases in EG was significantly superior to those either in CG1 or in CG2,with their LSRS improved obviously(P<0.05),as shown in each time window during the treating course.However,there was no such significantly statistic significance between CG1 and CG2(P>0.05).Conclusion It can be concluded from this trial that acupuncture,herbal medicine and the therapy combined acupuncture with herbal medicine are all effective on deafness with low speech recognition scores,guided under the treating principle of invigorating the Spleen and nourishing the Kidney.However,the most superior therapeutic effect should be brought about by the combined therapy.
