摘　　要：目的:探讨头针带针状态下行康复训练治疗中风后肩手综合征Ⅰ~Ⅱ期的临床治疗效果。方法:选取2020年1月—2021年12月于山西省针灸医院就诊的中风后肩手综合征Ⅰ~Ⅱ期患者110例,按随机数字表划分为治疗组和对照组,各55例。两组均给予常规治疗,对照组联合康复训练,治疗组则给予头针带针状态下行康复训练。比较两组治疗前后手掌情况(手掌体积、手掌厚度)、肩关节活动度(内收、外展、前屈、后伸、旋前和旋后)、上肢静脉最大血流速度(腋静脉、肘静脉和肱静脉)、上肢运动功能评分(Fugl-Meyer量表)和生活质量评分(SS-QOL评分),统计两组患者临床治疗总有效率。结果:治疗组总有效率为94.55%(52/55),明显高于对照组的81.82%(45/55),差异具有统计学意义(P<0.05)。两组治疗后手掌体积、手掌厚度均较治疗前明显降低(P<0.05),肩关节活动度、上肢静脉最大血流速度、上肢运动功能评分和SS-QOL评分均较治疗前明显改善(P<0.05),且治疗组治疗后的上述各项指标均明显优于对照组(P<0.05)。结论:头针带针状态下行康复训练可有效改善中风后肩手综合征患者手掌肿胀情况,提高肩关节灵活度,改善上肢静脉血液循环,促进上肢功能恢复,并提高患者生活质量。Objective:To explore the clinical therapeutic effect of scalp acupuncture with retaining needles and rehabilitation training in treatment of post-stroke shoulder-hand syndrome(SHS)of stageⅠ-Ⅱ.Methods:A total of 110 patients with post-stroke SHS of stageⅠ-Ⅱwere randomly divided into the treatment group and the control group,with 55 patients in each group.Both groups were given routine treatment,the control group was treated with rehabilitation training;on which basis,the treatment group was also treated with scalp acupuncture with retaining needling.The condition of palms(palm volume,palm thickness),range of motion(ROM)of shoulder joints(adduction,abduction,flexion,extension,pronation and supination),maximum blood flow velocity of upper extremity veins(axillary vein,cubital vein,brachial vein),upper extremity motor function(Fugl-Meyer scale)score and stroke specific quality of life(SS-QOL)score were compared between before and after the treatment in the two groups.Clinical efficacy was also compared between the two groups.Results:The total effective rate was 94.55%(52/55)in the treatment group,which was significantly higher than 81.82%(45/55)in the control group(P<0.05).The palm volume and the palm thickness were significantly decreased(P<0.05),and the range of motion of shoulder joint,maximum venous blood flow velocity of upper extremity,upper extremity motor function score and SS-QOL score were significantly improved after the treatment compared with those before the treatment in the two groups(P<0.05),and the improvements of the above indicators were more significant in the treatment group than those in the control group(P<0.05).Conclusion:Scalp acupuncture with retaining needles and rehabilitation training can effectively improve the swelling of the palms of the patients with post-stroke SHS,improve the flexibility of the shoulder joint,improve the venous blood circulation of the upper extremities,promote the recovery of upper extremity function,and improve the life quality of the patients.
