摘　　要：环状弹性纤维溶解性巨细胞肉芽肿(AEGCG)是一种罕见的肉芽肿性皮肤病。本文报道1例67岁男性患者,面、颈、肩、背及上肢环形斑块,轻度瘙痒1年余。皮肤组织病理学检查示真皮浅中层以淋巴细胞、组织细胞和多核巨细胞为主的肉芽肿性炎症浸润,局部可见较多嗜酸性粒细胞。弹性纤维染色示肉芽肿区弹性纤维缺如,可见多核巨细胞吞噬弹性纤维现象。阿辛蓝染色阴性。结合患者临床表现和组织病理检查,诊断为AEGCG。本病的病因及发病机制不明,非曝光部位也可出现不典型表现,通常与系统性损害有关,目前尚无特效治疗。Annular elastolytic giant cell granuloma(AEGCG)is a rare granulomatous skin condition.We present a case of a 67⁃year⁃old man with annular plaques on the face,neck,shoulder,back and upper limbs,and mildly pruritis exceeding one year.Histopathological examination demonstrated granulomatous in⁃flammatory infiltration of lymphocytes,histiocytes and multinuclear giant cells in the middle and the upper dermis,with more local eosinophils.Elastic fiber staining showed that elastic fibers were absent in granuloma area and were engulfed by multinucleated giant cells.Based on the clinical and histopathological findings,a diagnosis of AEGCG was made.The etiology and pathogenesis of this condition are unclear,and atypical man⁃ifestations of non⁃exposed areas can also occur.It is usually related to systemic diseases lacks specific treat⁃ment at present.
