仙灵骨葆胶囊联合针灸、膝关节封闭对退行性膝骨关节炎患者的临床疗效

摘　　要：目的考察仙灵骨葆胶囊联合针灸、膝关节封闭对退行性膝骨关节炎患者的临床疗效。方法110例患者随机分为对照组(52例)和观察组(58例),对照组采用针灸、膝关节封闭,观察组在对照组基础上加用仙灵骨葆胶囊,疗程4周。检测炎症因子(IL-1、IL-6、TNF-α、ESR)、骨代谢相关指标(TRACP、CTX-Ⅰ、BGP、OPG、BALP)、疼痛因子(5-HT、SP、PGE2)、VAS评分、Lyshom评分、WOMAC评分、膝关节活动度、中医证候评分、不良反应发生率变化。结果治疗后,2组炎症因子、疼痛因子、TRACP、CTX-Ⅰ、VAS评分、WOMAC评分、中医证候评分降低(P<0.05),BGP、OPG、BALP、Lyshom评分、膝关节活动度升高(P<0.05),以观察组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论仙灵骨葆胶囊联合针灸、膝关节封闭可安全有效地抑制退行性膝骨关节炎患者疼痛因子、炎症因子表达,调节骨代谢,改善膝关节功能。AIM To investigate the clinical effects of Xianling Gubao Capsules combined with acupuncture and intra-articular injection on patients with degenerative knee osteoarthritis.METHODS One hundred and ten patients were randomly assigned into control group(52 cases)for 4-week intervention of acupuncture and knee joint blocking,and observation group(58 cases)for 4-week intervention of Xianling Gubao Capsules,acupuncture and knee joint blocking.The changes in inflammatory factors(IL-1,IL-6,TNF-α,ESR),bone metabolism-related indices(TRACP,CTX-Ⅰ,BGP,OPG,BALP),pain factors(5-HT,SP,PGE2),VAS score,Lyshom score,WOMAC score,knee joint activity,TCM syndrome scores and incidence of adverse reactions were detected.RESULTS After the treatment,the two groups displayed decreased inflammatory factors,pain factors,TRACP,CTX-I,VAS score,WOMAC score,TCM syndrome scores(P<0.05),and increased BGP,OPG,BALP,Lyshom score,knee joint activity(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with degenerative knee osteoarthritis,Xianling Gubao Capsules combined with acupuncture and intra-articular injection can inhibit the expressions of pain factors and inflammatory factors,regulate bone metabolism,and improve knee joint functions.
