聚焦解决模式对初产妇负性情绪及分娩结局影响的Meta分析

摘　　要：目的 :评价聚焦解决模式对初产妇负性情绪及分娩结局的影响。方法:计算机检索中国知网、万方数据库、维普网、中国生物医学文献数据库、PubMed、WebofScience、CochraneLibrary和CINAHL等数据库,采用RevMan 5.4软件进行Meta分析。结果:纳入26篇文献,共计3527例初产妇。与常规护理相比,聚焦解决模式能够减轻初产妇的焦虑、抑郁情绪,聚焦解决模式组的自然分娩率和新生儿1 min Apgar评分显著高于对照组,其新生儿窒息率低于对照组,尚不能认为该模式可改善初产妇产后出血的情况。结论:聚焦解决模式能够减轻初产妇的焦虑、抑郁情绪,提高自然分娩率、新生儿1 min Apgar评分,降低新生儿窒息率,暂未发现可改善产后出血情况,有待多中心、大样本、设计严谨的研究去验证。Objective:To evaluate the effects of Solution-Focused Brief Therapy on negative emotions and childbirth outcomes of primiparas by Meta-analysis.Methods:We searched databases such as CNKI,WanFang Data,VIP,SinoMed,PubMed,Web of Science,Cochrane Library and CINAHL.Then we performed Meta-analysis using RevMan 5.4 software.Results:Twenty-six studies were finally included,including 3527 primiparas.Meta-analysis results show that nursing based on Solution-Focused Brief Therapy can reduce the anxiety and the depression of primiparas;the rate of spontaneous childbirth and the 1 min Apgar score in the solution-focused group was significantly higher than that in the control group,the rate of neonatal asphyxia in the solution-focused group was lower than that in the control group,it is not yet considered that the Solution-Focused Brief Therapy can improve postpartum hemorrhage in primiparas situation.Conclusion:Solution-Focused Brief Therapy can reduce the anxiety and depression of primiparas,and can improve the natural delivery rate,1 min Apgar score,and neonatal asphyxia rate of primiparas.It has not been found to improve postpartum hemorrhage in primiparas,which needs to be verified by multi-center,large sample and well-designed studies.
