腹腔镜穿孔修补术治疗上消化道溃疡穿孔的效果分析

摘　　要：目的:探讨腹腔镜穿孔修补术治疗上消化道溃疡穿孔的效果。方法:选取2020年1—12月北京市昌平区中医医院收治的90例上消化道溃疡穿孔患者作为研究对象,根据双盲法分为研究组与对照组,各45例。对照组给予开腹穿孔修补术,研究组给予腹腔镜穿孔修补术。比较两组治疗效果。结果:术后,研究组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,CD8^(+)水平低于对照组,差异有统计学意义(P=0.000)。研究组术后各项生活质量评分高于对照组,差异有统计学意义(P=0.000)。研究组术中出血量、术后引流量少与对照组,手术时间、肛门排气时间、住院时间均短于对照组,差异有统计学意义(P=0.000)。研究组并发症发生率低于对照组,差异有统计学意义(P=0.036)。结论:腹腔镜穿孔修补术治疗上消化道溃疡穿孔的效果显著,可改善患者免疫功能及生活质量,促进病情恢复,减少并发症。Objective:To investigate the effect of laparoscopic perforation repair for treating upper digestive tract ulcer perforation.Methods:A total of 90 patients with upper digestive tract ulcer perforation treated in Beijing Changping Hospital of Chinese Medicine from January 2020 to December 2020 were selected as the study subjects.The patients were divided into study group and control group by the double-blind method,with 45 cases in each group.The control group received laparotomy perforation repair.The study group received laparoscopic perforation repair.The therapeutic effect was compared between the two groups.Results:After surgery,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the study group were higher than those in the control group,while the levels of CD8^(+)in the study group were lower than those in the control group,with statistically significant difference(P=0.000).After surgery,the life quality scores in the study group were higher than those in the control group,and the difference was statistically significant(P=0.000).The intraoperative blood loss and postoperative drainage volume in the study group were less than those in the control group,the operation time,anal exhaust time,and hospital stay in the study group were shorter than those in the control group,and the difference was statistically significant(P=0.000).The complication incidence in the study group was lower than that in the control group,and the difference was statistically significant(P=0.036).Conclusion:Laparoscopic perforation repair for treating upper digestive tract ulcer perforation can obtain remarkable effect,which can improve immune function and quality of life,promote recovery,and reduce complications.
