摘　　要：目的:探讨社区慢性病管理模式在老年高血压患者中的应用价值。方法:选取2019年12月—2020年11月在金华市江南街道社区卫生服务中心就诊的102例老年高血压患者作为研究对象,按照掷硬币法分为对照组与观察组,各51例。对照组实施常规社区管理,观察组实施社区慢性病管理,比较两组疾病知识掌握度、治疗依从性情况、血压水平、满意度。结果:观察组疾病知识掌握度高于对照组,差异有统计学意义(P=0.012)。干预前,两组治疗依从性比较,差异无统计学意义(P=0.510);干预后,两组治疗依从性水平均高于干预前,且观察组高于对照组,差异有统计学意义(P=0.027)。干预前,两组舒张压、收缩压比较,差异无统计学意义(P>0.05);干预后,两组舒张压、收缩压均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组满意度高于对照组,差异有统计学意义(P=0.014)。结论:社区慢性病管理模式在老年高血压患者中的应用效果显著,可提高患者疾病知识掌握度、治疗依从性及满意度,改善血压水平。Objective:To explore the application value of community chronic disease management model in the elderly patients with hypertension.Methods:A total of 102 elderly hypertensive patients who were treated at the Jiangnan Street Community Health Service Center in Jinhua City from December 2019 to November 2020 were selected as the study subjects,and they were divided into control group and observation group according to the coin toss method,with 51 cases in each group.The control group implemented conventional community management,and the observation group implemented community chronic disease management.The disease knowledge,treatment compliance,blood pressure level,and satisfaction in two groups were compared.Results:The disease knowledge mastery in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.012).Before the intervention,there was no significant difference in treatment compliance between the two groups(P=0.510).After the intervention,the level of treatment compliance in two groups was higher than that before the intervention,and the observation group was higher than the control group,and the difference was statistically significant(P=0.027).Before the intervention,there was no significant difference in diastolic blood pressure and systolic blood pressure between the two groups(P>0.05).After the intervention,the diastolic blood pressure and systolic blood pressure in two groups were lower than those before the intervention,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The satisfaction in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.014).Conclusion:The application effect of community chronic disease management model is significant in elderly hypertensive patients,which can improve the patient's disease knowledge mastery,treatment compliance and satisfaction,and ameliorate blood pressure levels.
