摘　　要：目的探讨经尺骨鹰嘴截骨入路Herbert螺钉治疗肱骨远端冠状面骨折的临床疗效。方法经尺骨鹰嘴截骨入路应用Herbert螺钉治疗肱骨远端冠状面骨折8例,按肱骨小头骨折Bryan-Morrey分型均为第Ⅳ型,按Dubberley分型:ⅡA型2例,ⅢA型5例,ⅢB型1例。结果随访8~20个月,骨折均愈合良好,采用改良Cassebaum评分优6例,良2例。结论尺骨鹰嘴截骨入路是治疗肱骨远端冠状面骨折的较好手术入路,Herbert螺钉可以牢固固定骨折。Objective To investigate the clinical effect of Herbert screw through olecranon osteotomy in the treatment of coronal plane fracture of distal humerus.Methods Eight cases of coronal fracture of distal humerus were treated with Herbert screw through olecranon osteotomy approach.All fractures were classified as typeⅣaccording to Bryany-Morrey classification of humerus small head fracture.Among them two cases were classified as typeⅡA,five cases were classified as typeⅢA,and one case was classified as typeⅢB according to Dubberley classification.Results After 8 to 20 months of follow-up,all fractures healed well.The modified Cassebaum score was excellent in six cases and good in two cases.Conclusion The olecranon osteotomy approach is a good surgical approach for the treatment of the coronal plane fracture of the distal humerus,and Herbert screws can be used to firmly fix the fracture.
