激光人工皱缩预处理优质囊胚和非优质囊胚对妊娠结局的影响

摘　　要：目的探讨激光人工皱缩预处理优质囊胚和非优质囊胚对妊娠结局的影响。方法1016例囊胚移植女性患者根据囊胚分级系统分为优质囊胚(A)组758例和非优质囊胚(B)组258例,再根据囊胚玻璃化冷冻前是否行激光人工皱缩,A组分为行激光人工皱缩亚组(A1组,306例)和未行激光人工皱缩亚组(A2组,452例),B组分为行激光人工皱缩亚组(B1组,140例)和未行激光人工皱缩亚组(B2组,118例),比较两组及其亚组妊娠结局相关指标。结果A组临床妊娠率和活产率高于B组(P<0.01)。A1组临床妊娠率和活产率高于A2组(P<0.05)。B1组和B2组妊娠结局相仿(P>0.05)。结论激光人工皱缩预处理可显著提高优质囊胚冷冻复苏后的妊娠结局,而对非优质囊胚则效果不显著。Objective To explore the effects of laser artificial shrinkage pretreatment of high-quality blastocysts and non-high-quality blastocysts on pregnancy outcomes.Methods According to the blastocyst grading system,1016 female patients were divided into groups of A(high-quality blastocyst,758 cases)and B(non-high-quality blastocyst,258 cases).According to whether laser artificial shrinkage was performed before blastocyst vitrification,group A was subdivided into groups of A1(with laser artificial shrinkage,306 cases)and A2(without laser artificial shrinkage,452 cases),and group B was subdivided into groups of B1(with laser artificial shrinkage,140 cases)and B2(without laser artificial shrinkage,118 cases).Pregnancy outcomes-related indicators were compared between the two groups and their subgroups.Results The clinical pregnancy rate and live birth rate in group A were higher than those in group B(P<0.01).The clinical pregnancy rate and live birth rate in group A1 were also higher than those in group A2(P<0.05),which in group B1 were similar to those in group B2(P>0.05).Conclusion Laser artificial shrinkage pretreatment can obviously improve the pregnancy outcomes after cryo-resuscitation of high-quality blastocysts,which is not effective in the non-high-quality blastocysts.
