摘　　要：目的探讨不同类型药物性肝损伤(DILI)患者血清细胞角蛋白18-M30(CK18-M30)和CK18-M65水平及其临床意义。方法回顾性分析40例肝细胞型DILI患者(HDILI组)、11例胆汁淤积型DILI患者(CDILI组)和18例肝细胞型药物性肝衰竭患者(HDILF组)的临床资料,另以43例健康志愿者作为对照组。比较各组血清CK18-M30、CK18-M65水平,绘制ROC曲线评估各指标的诊断价值。结果血清CK18-M30和CK18-M65水平在CDILI组、HDILI组、HDILF组、对照组间均有统计学差异(P<0.01);其中,HDILF组血清CK18-M30、CK18-M65水平最高。CK18-M30、CK18-M65诊断肝细胞型DILI的AUC分别为0.895、0.939。CK18-M30、CK18-M65诊断胆汁淤积型DILI的AUC分别为0.817、0.836。结论血清CK18-M30和CK18-M65水平与DILI的类型和严重程度有关。Objective To investigate serum levels and clinical significance of CK18-M30 and CK18-M65 in the patients with drug-induced liver injury(DILI).Methods The clinical data of 40 patients with hepatocellular DILI(group HDILI),11 patients with cholestasis DILI(group CDILI)and 18 patients with hepatocellular drug-induced liver failure(group HDILF)were retrospectively analyzed.Another 43 healthy people were taken as the controls(group C).Serum levels of CK18-M30,CK18-M65 were compared among four groups.The diagnostic and prognostic value of above indexes were evaluated using ROC curve analysis.Results Serum levels of CK18-M30,CK18-M65 were significantly different among groups of C,CDILI,HDILI and HDILF(P<0.01).Serum levels of CK18-M30 and CK18-M65 were the highest in group HDILF.The AUC of CK18-M30,CK18-M65 in diagnosis of hepatocellular DILI was 0.895,0.939,respectively.The AUC of CK18-M30,CK18-M65 in diagnosis of cholestasis DILI was 0.817,0.836,respectively.ConclusionSerum levels of CK18-M30 and CK18-M65 are related to the type and severity of DILI.
