摘 要:目的探究双下肢全长负重位拼接摄影(DRFSF)对全膝关节置换术(TKA)术前计划及术后疗效评估的价值。方法选取2021年1~12月于广州中医药大学惠州医院接受双下肢全长拼接摄影并行TKA的患者共50例。对所有患者应用X光机数字化透视摄影X射线系统DR,观察资料。结果DRFSF摄片图像质量满足临床要求。术前显示膝关节内翻畸形24例、外翻畸形15例、存在膝关节骨性关节炎的患者11例。TKA后较于术前患者下肢骨骼形态及生理力线位置、髋-膝-踝角及股骨下角明显改善(P<0.05)。结论双下肢DRFSF技术能够更为精准、清晰地显示双下肢全长,保障TKA开展,应用价值高。Objective To explore value of Digital Radiography Full-length Stitching Film(DRFSF)in preoperative planning and postoperative evaluation of total knee arthroplasty(TKA).Methods The paper chose 50 patients(pts)received full-length splicing photography of both lower limbs and TKA in Guangzhou University of Chinese Medicine Huizhou Hospital from January to December 2021,treated with X-ray machine digital fluoroscopy X-ray system DR.Observation data were automatically spliced by DXRay Diagnost software.Morphology,physiological angle and physiological force line of whole lower limb was observed and analyzed.Morphological structure changes,lower limb force line and tibiofemoral angle were measured accurately before and after the knee joint replacement for comparative analysis.Results Photos of 50 patients with DRFSF of both lower limbs can be clearly displayed on the same film,with excellent splicing quality,and image quality can meet clinical requirements.There were 24 cases of varus deformity of knee joint,15 cases of valgus deformity of knee joint,and 11 cases of pts without varus deformity of knee joint and only with osteoarthritis of knee joint.After TKA,lower limb bone morphology and physiological force line position of pts patients was significantly better than before operation,and hip knee ankle angle and lower femur angle was significantly better after operation than before operation,with statistically significant difference(P<0.05)Conclusion DRFSF technology of both lower limbs can more accurately and clearly display full length of both lower limbs,better ensure development of TKA,provide better clinical basis for lower limb force line,tibial angle and curative effect evaluation,which has high application value and is worthy promotion.
