百色市医养结合“双向联合体”模式的探讨

摘　　要：构建医养结合服务体系是积极应对人口老龄化的重要措施,百色市成立首个医养结合联合体“颐养园”,医院与多家公建民营养老机构联合,通过医生、护士、药师、康复人员等多专业人员介入,形成定期巡诊、开设内部门诊、家庭服务病床、转诊绿色通道等模式,充分发挥中医及民族医药优势,实行分级养老,合理分配医疗资源,初步打造“医疗-康复-护理-养老”结合,建立医院与养老机构“双向结合、联合运行”的特色联合体运行模式,该工作模式及相关制度为医养结合工作的开展提供了实践经验,为构建医养结合模式及对促进解决养老问题提供思路。Constructing integrated medical and nursing service system is an important measure to respond to aging population actively.Baise City has established the first integrated medical and nursing union"Ealily Living Garden".Hospitals has combined with many public and civilian nutrition institutions for the elderly,with intervention of doctors,nurses,pharmacists,rehabilitation staff and other professionals,formed modes with regular rounds,internal outpatient service,family service beds,referral green channel and etc,giving full play to advantages of traditional Chinese medicine and ethnomedicine,to implement hierarchical pension,allocate medical resources reasonably,create"medical-rehabilitation-nursing-elderly care"combination initially,and establish a characteristic joint operation mode of"two-way combination and joint operation"between hospitals and elderly care institutions.The working mode and relevant systems provide practical experience for work of integrating medical care and elderly care,and provide ideas for constructing integration of medical care and elderly care and promoting solution of elderly care issue.
