早期胸腔积液引流联合醋酸泼尼松片治疗结核性渗出性胸膜炎患者的效果评价

摘　　要：目的探讨早期胸腔积液引流联合醋酸泼尼松治疗结核性渗出性胸膜炎的疗效。方法140例结核性渗出性胸膜炎患者,依据治疗方法的不同分为对照组和观察组,每组70例。对照组采取常规抗结核药物治疗,观察组在对照组基础上实施早期胸腔积液引流联合醋酸泼尼松片治疗。对比两组患者临床治疗效果及症状改善时间。结果观察组患者治疗总有效率94.29%高于对照组的77.14%,差异有统计学意义(P<0.05)。观察组胸腔积液吸收时间(15.36±2.13)d、结核中毒症状消失时间(9.53±2.41)d和体温恢复时间(3.68±1.12)d均短于对照组的(18.79±2.51)、(15.29±3.25)、(5.36±1.25)d,差异具有统计学意义(P<0.05)。结论在口服抗结核药物基础上进行早期胸腔积液引流联合醋酸泼尼松片治疗结核性渗出性胸膜患者,可以减少胸膜肥厚或粘连发生,值得临床推广应用。Objective To discuss the efficacy of early pleural effusion drainage combined with prednisone acetate tablets in the treatment of patients with tuberculous exudative pleurisy.Methods A total of 140 patients with tuberculous exudative pleurisy were divided into control group and observation group according to different treatment methods,with 70 cases in each group.The control group was treated with conventional antituberculosis drugs,and the observation group was treated with early pleural effusion drainage and prednisone acetate tablets on the basis of the control group.The clinical therapeutic effect and symptom improvement time of the two groups were compared.Results The total effective rate in the observation group was 94.29%,which was higher than that of 77.14%in the control group,and the difference was statistically significant(P<0.05).The observation group had absorption time of pleural effusion of(15.36±2.13)d,disappearance time of tuberculosis symptom of(9.53±2.41)d and recovery time of body temperature of(3.68±1.12)d,which were shorter than those of(18.79±2.51),(15.29±3.25)and(5.36±1.25)d in the control group,and the differences were statistically significant(P<0.05).Conclusion Early pleural effusion drainage combined with prednisone acetate tablets on the basis of oral antituberculosis drugs for tuberculous exudative pleura patients can reduce the occurrence of pleural hyperplasia or adhesion,which is worthy of clinical promotion and application.
