摘  要:目的 分析血清巨噬细胞炎症蛋白1α(MIP-1α)、白细胞介素-13(IL-13)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平变化与重症哮喘患者肺功能的相关性。方法 选取兰考第一医院2019年6月至2021年5月收治的101例重症哮喘患者,另选取同期95例健康体检者作为研究对象。采用酶联免疫检测法检测血清MIP-1α、IL-13、IL-6、TNF-α水平;采用肺功能检测仪检测呼气峰流速(PEF)、最大呼气中段流量(MMEF)、第1秒用力呼气容积(FEV_(1))。比较治疗前及治疗后1、3、7 d重症哮喘患者血清MIP-1α、IL-13、IL-6、TNF-α水平与PEF、MMEF、FEV_(1)水平,并分析其相关性。结果 治疗后1、3、7 d,重症哮喘患者血清MIP-1α、IL-13、IL-6、TNF-α水平低于治疗前,且呈下降趋势(P<0.05);治疗前及治疗后1、3、7 d,重症哮喘患者血清MIP-1α、IL-13、IL-6、TNF-α水平高于健康体检者(P<0.05);治疗后1、3、7 d,重症哮喘患者PEF、MMEF、FEV_(1)水平均高于治疗前,且呈上升趋势(P<0.05);治疗前及治疗后1、3、7 d,重症哮喘患者PEF、MMEF、FEV_(1)水平均低于健康体检者(P<0.05);重症哮喘患者血清MIP-1α、IL-13、IL-6、TNF-α水平与PEF、MMEF、FEV_(1)水平均呈负相关关系(P<0.05)。结论 血清MIP-1α、IL-13、IL-6、TNF-α水平越高,提示重症哮喘患者肺功能越差,临床可通过检测患者的血清MIP-1α、IL-13、IL-6、TNF-α水平变化评估症哮喘患者的预后,并指导临床治疗方案的制定,具有良好应用价值。Objective To analyze the correlation of serum levels of macrophage inflammatory protein 1α(MIP-1α),interleukin-13(IL-13),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)with lung function in patients with severe asthma.Methods A total of 101 patients with severe asthma in Lankao First Hospital from June 2019 to May 2021 were selected,and 95 healthy physical examination during the same period were selected.The serum levels of MIP-1α,IL-13,IL-6 and TNF-αbefore and after 1,3,7 days of treatment were detected by enzyme-linked immunosorbent assay.Peak expiratory flow rate(PEF),maximum mid-expiratory flow(MMEF),and forced expiratory volume in the first second(FEV_(1))before and after 1,3,7 days of treatment were detected by pulmonary function tester.The correlations of serum levels of MIP-1α,IL-13,IL-6,TNF-αwith PEF,MMEF,FEV_(1) in patients with severe asthma were analyzed.Results Compared with before treatment,the serum levels of MIP-1α,IL-13,IL-6,and TNF-αin patients with severe asthma were lower after 1,3,and 7 days of treatment,and showed a downward trend(P<0.05).Serum levels of MIP-1α,IL-13,IL-6,and TNF-αin patients with severe asthma were higher than those in healthy physical examination before and after 1,3,7 days of treatment(P<0.05).Compared with before treatment,the levels of PEF,MMEF,FEV_(1) in patients with severe asthma were higher after 1,3,and 7 days of treatment,and showed an upward trend(P<0.05).Levels of PEF,MMEF,FEV_(1) in patients with severe asthma were lower in healthy physical examination before and after 1,3,7 days of treatment(P<0.05).Serum levels of MIP-1α,IL-13,IL-6,TNF-αwere negatively correlated with levels of PEF,MMEF,FEV_(1) in patients with severe asthma(P<0.05).Conclusion Serum levels of MIP-1α,IL-13,IL-6,TNF-αare higher,the pulmonary function of patients with severe asthma are worse.The serum of patients can be detected clinically MIP-1α,IL-13,IL-6,TNF-α.Serum levels of MIP-1α,IL-13,IL-6,TNF-αcan be detected to evaluate the prognosis of asthmatic patients and guid
