摘　　要：目的 探讨鼻内镜扩大鼻窦开放术(EESS)对慢性鼻窦炎伴鼻息肉患者额窦引流及嗅觉功能的影响。方法 选取驻马店市第一人民医院2020年2月至2021年9月收治的88例慢性鼻窦炎伴双侧鼻息肉患者,根据手术方式分为观察组(46例)和对照组(42例)。对照组接受功能性内镜鼻窦术(FESS)治疗,观察组接受EESS治疗,两组均随访1 a,比较术后6个月及术后1 a嗅觉功能、额窦通畅情况以及并发症发生率和临床满意度。结果 术后6个月和术后1 a,两组Sniffin’ Sticks嗅觉测试评分均高于术前(P<0.05),术后6个月,观察组Sniffin’ Sticks嗅觉测试评分高于对照组(P<0.05);两组均双侧额窦开放,观察组术后6个月额窦通畅率(95.65%)高于对照组(76.19%)(P<0.05);随访期间,两组均未出现鼻腔黏膜干燥、鼻出血、脑脊液漏以及萎缩性鼻炎等并发症;术后1 a,观察组临床满意度(93.48%)高于对照组(78.57%)(P<0.05)。结论 EESS治疗慢性鼻窦炎伴双侧鼻息肉安全性良好,可有效改善嗅觉功能,提高额窦通畅率和临床满意度。Objective To investigate the effect of extended endoscopic sinus surgery(EESS)on frontal sinus drainage and olfactory function in patients with nasal polyps.Methods A total of 88 patients with nasal polyps and allergy who were admitted to the First People’s Hospital of Zhumadian from February 2020 to September 2021 were divided into the observation group(46 cases)and the control group(42 cases)according to surgical methods.The control group was treated with functional endoscopic sinus surgery(FESS),while observation group was treated with EESS.Both groups were followed up for 1 year.The olfactory function,frontal sinus patency,incidence of complications and clinical satisfaction were compared between the two groups at 6 months and 1 year after surgery.Results At 6 months and 1 year after surgery,score of Sniffin’Sticks test were higher than those before surgery in both groups,and the differences were statistically significant(P<0.05).At 6 months after surgery,score of Sniffin’Sticks test in observation group was significantly higher than that in control group(P<0.05).Both groups underwent bilateral frontal sinus opening.At 6 months after surgery,frontal sinus patency rate in observation group(95.65%)was higher than that in control group(76.19%)(P<0.05).During surgery and 1-year follow-up,there were no complications(dry nasal mucosa,epistaxis,cerebrospinal fluid leakage,atrophic rhinitis)in either group.At 1 year after surgery,clinical satisfaction rate in observation group(93.48%)was higher than that in control group(78.57%)(P<0.05).Conclusion EESS is safe in the treatment of patients with chronic sinusitis and bilateral nasal polyps.Besides,it can effectively improve olfactory function,frontal sinus patency rate and clinical satisfaction.
