全膝关节置换术后患者膝关节松弛程度与体重指数的关系及满意度调查

摘　　要：目的 明确全膝关节置换术(TKA)后患者膝关节松弛程度的变化、松弛程度变化可能的危险因素以及松弛程度对满意度的影响。方法 回顾性分析,收集2019年1—12月在郑州大学第一附属医院骨科就诊的因膝关节骨性关节炎接受TKA的65例患者的临床资料,共74膝。术后进行2 a随访调查。分别于术前(T_(0))、术后3个月(T_(1))、术后6个月(T_(2))、术后12个月(T_(3))、术后24个月(T_(4)),采用视觉模拟评分法(VAS)评估疼痛情况,采用膝关节功能评分(HSS)评价膝关节功能,采用稳定性检测评价患者膝关节松弛度变化,记录患者各随访阶段身高体重并计算体重指数,自制手术满意度调查问卷调查患者满意度,并分析膝关节松弛度变化对患者满意度的影响。结果 较术前,患者术后各时点VAS评分逐渐降低,HSS评分逐渐升高,Lysholm稳定性评分逐渐升高,前抽屉实验结果水平逐渐升高,术后各时点活动度逐渐增加,重复测量显示差异有统计学意义(P<0.05);线性分析显示,Lysholm稳定性评分、前抽屉实验结果水平与患者体重指数呈正相关(r>0,P<0.05);术后随访各时点,患者满意度评分有一定升高,但各时点间重复测量显示差异无统计学意义(P>0.05)。结论 TKA后患者疼痛感减轻,膝关节功能改善,但膝关节存在一定程度松弛,可能与患者体重指数有关,但这种术后松弛程度并不会降低患者对术后膝关节活动功能的满意度。Objective To clarify the changes of knee joint relaxation degree after total knee arthroplasty(TKA),the possible risk factors of knee joint relaxation degree changes and the influence of knee joint relaxation degree on satisfaction.Methods A retrospective analysis was performed to collect the clinical data of 65 patients with knee osteoarthritis who underwent TKA in the Department of Orthopaedics of the First Affiliated Hospital of Zhengzhou University from January to December 2019.After 2 years of follow-up,the investigation was carried out.Visual analogue scale(VAS)was used to evaluate pain before operation(T_(0)),3 months after operation(T_(1)),6 months after operation(T_(2)),12 months after operation(T_(3))and 24 months after operation(T 4).Knee joint function score(HSS)was used to evaluate knee joint function.Stability test was used to evaluate the change of knee joint relaxation.The height and weight of patients in each follow-up stage were recorded and body mass index was calculated.Self-made operation satisfaction questionnaire was used to investigate patient satisfaction,and the effect of knee joint relaxation change on patient satisfaction was analyzed.Results Compared with before operation,the VAS score decreased gradually,the HSS score increased gradually,the Lysholm stability score increased gradually,the level of anterior drawer test results increased gradually,and the activity increased gradually at each time point after operation,and repeated measures showed that the difference was statistically significant(P<0.05).Linear analysis showed that Lysholm stability score and anterior drawer test results were positively correlated with body mass index(r>0,P<0.05).At each time point of postoperative follow-up,the patient satisfaction score increased to a certain extent,but there was no statistical difference in the repeated measurement at each time point(P>0.05).Conclusion After TKA,the patient’s pain was relieved and the knee joint function was improved,but there was a certain degree of knee joint rel
