摘　　要：目的 了解某医院检验危急值的报告情况,为进一步规范管理危急值提供科学依据。方法 利用东华信息管理系统(LIS)回顾性分析2021年该院检验危急值发生率、危急值科室分布情况、危急值项目分布情况及危急值报告合格率。结果 2021年全年危急值发生率为1.07%。危急值数量较多的科室分别为血液科(15.47%)、感染科(9.66%)、肾病科(8.55%)、儿内科(7.56%)、重症医学科(7.00%)。危急值项目主要集中于血小板(23.55%)、白细胞(18.10%)、尿素(7.65%)、血钙(7.32%)、血钾(5.98%)5项。全年危急值确认合格率平均为87.43%,其中1—6月平均危急值确认合格率为78.64%,7—12月平均危急值确认合格率为96.27%;住院患者危急值报告合格率为90.1%,急诊报告合格率为93.0%,门诊报告合格率为68.9%;各科室间危急值确认合格率差异有统计学意义(P<0.05)。结论 在危急值管理过程中要定期进行危急值报告回顾与分析并发现问题,以便于危急值的持续改进。Objective To analyze the situation of critical value report of a hospital,so as to provide a scientific evidence for further standardized management of critical value.Methods Donghua Information Management System(LIS)was used to retrospectively analyze the incidence of critical value,the distribution of critical value departments,the distribution of critical value items and the qualified rate of critical value report in the hospital in 2021.Results The 2021 incidence of critical value was 1.07%.The departments with more critical values were hematology(15.47%),infection(9.66%),nephrology(8.55%),pediatrics(7.56%)and intensive care unit(7.00%).The items of critical value mainly concentrated in platelets(23.55%),white blood cell(18.10%),blood urea nitrogen(7.65%),blood calcium(7.32%),blood potassium(5.98%).The average critical value qualification rate was 87.43%,from January to June was 78.64%,from July to December was 96.27%.The critical value qualification rate of inpatient was 90.1%,the emergency was 93.0%,and the outpatient was 68.9%.There was statistically significant difference in the qualification rate of critical value confirmation among departments(P<0.05).Conclusion In the process of critical value management,it is necessary to regularly review and analyze the problems,so as to facilitate the continuous improvement of critical value.
