中药健脾解毒方联合化疗对高危弥漫大B细胞淋巴瘤患者的临床疗效

摘　　要：目的探讨中药健脾解毒方联合化疗对高危弥漫大B细胞淋巴瘤患者的临床疗效。方法选取南阳市中医院2020年1月至2022年10月收治的80例高危弥漫大B细胞淋巴瘤患者,根据建档顺序分为对照组和观察组,各40例。对照组患者接受R-CHOP化疗方案治疗,观察组患者在对照组基础上加用健脾解毒方。比较两组患者疗效、中医症状持续时间、免疫功能、炎症反应水平以及不良反应发生率。结果两组患者客观缓解率及疾病控制率相比,观察组患者均较高(P<0.05);治疗后,两组患者各中医症状持续时间相比,观察组均较短(P<0.05);治疗后,两组患者免疫功能均有所提高,且观察组免疫功能较强(P<0.05);治疗后,两组血清白细胞介素-2(IL-2)水平均升高,且观察组较高;两组血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平均降低,且观察组较低(P<0.05);两组总不良反应发生率相比,差异无统计学意义(P>0.05)。结论中药健脾解毒方联合化疗可通过调节高危弥漫大B细胞淋巴瘤患者免疫球蛋白、炎症细胞因子水平,从而提高患者的临床疗效。Objective To investigate the effect of Chinese spleen-strengthening and detoxification formula combined with chemotherapy in patients with high-risk diffuse large B-cell lymphoma.Methods Eighty patients with high-risk diffuse large B-cell lymphoma admitted to Nanyang Hospital of Traditional Chinese Medicine from January 2020 to October 2022 were selected and divided into a control group and an observation group according to the order of establishment,with 40 cases each group.Patients in the control group were given R-CHOP chemotherapy regimen,and patients in the observation group were treated with the Chinese spleen-strengthening and detoxification formula on the basis of control group.The efficacy,duration of TCM symptoms,immune function,level of inflammatory response and incidence of adverse reactions were compared between the two groups.Results After treatment,the duration of each TCM symptom was shorter in the observation group compared with the two groups(P<0.05).After treatment,the immune function of patients in both groups improved,and the immune function was stronger in the observation group(P<0.05).After treatment,the serum interleukin-2(IL-2)level increased in both groups,and it was higher in the observation group.Serum interleukin-6(IL-6)and interleukin-8(IL-8)levels were reduced,and it was lower in the observation group(P<0.05).There was no statistical difference in the incidence of total adverse reactions between the two groups(P>0.05).Conclusion The combination of Chinese spleen-strengthening and detoxification formula with chemotherapy can improve the clinical efficacy of patients with high-risk diffuse large B-cell lymphoma by regulating the levels of immunoglobulins and inflammatory cytokines.
