引导式健康教育及悬吊运动训练对痉挛型脑瘫患儿的干预效果

摘　　要：目的观察引导式健康教育(CE)联合悬吊运动训练(SET)在痉挛型脑瘫(SCP)患儿中的应用效果。方法选取2020年12月至2021年12月南阳市中心医院收治的82例SCP患儿作为研究对象,按照随机数字表法分为观察组和对照组,各41例。两组患者均接受常规护理,对照组接受CE干预,观察组在对照组的基础上接受SET干预。评价两组护理满意度、痉挛程度[综合痉挛量表(CSS)评分]、运动功能[粗大运动功能评估量表(GMFM)评分]、平均血流速度(Vm)、搏动指数(PI)、收缩期血流度(PSV)。结果干预前,两组GMFM、CSS、PI、PSV、Vm比较,差异无统计学意义(P>0.05);干预后,两组患儿CSS、PI、PSV、VM降低,GMFM评分升高,且观察组优于对照组(P<0.05)。观察组的护理满意度高于对照组(P<0.05)。结论SCP患儿经CE联合SET干预可改善患儿运动功能、痉挛程度、脑血流灌注情况,家属护理满意度高,临床干预效果好。Objective To observe the application effect of guided health education(CE)combined with suspension exercise training(SET)in children with spastic cerebral palsy(SCP).Methods A total of 82 children with SCP admitted to Nanyang Central Hospital from December 2020 to December 2021 were selected as the study subjects,and were randomly divided into an observation group and a control group,with 41 cases in each group.Both groups of patients received routine care,the control group received CE intervention,and the observation group received SET intervention on top of the control group.The nursing satisfaction,spasticity[comprehensive spasticity scale(CSS)score],motor function crude[motor function assessment scale(GMFM)score],mean blood flow velocity(Vm),pulsatile index(PI)and systolic blood flow(PSV)of both groups were evaluated.Results Before intervention,there was no statistically significant difference in GMFM,CSS,PI,PSV and Vm between the two groups(P>0.05).After intervention,the CSS,PI,PSV,VM of the two groups of children decreased,while the GMFM score increased,and the observation group was better than the control group(P<0.05).The nursing satisfaction of the observation group was higher than that of the control group(P<0.05).Conclusion The combination of CE and SET intervention in SCP patients can improve their motor function,spasticity and cerebral blood flow perfusion.Family nursing satisfaction is high,and the clinical intervention effect is good.
