发际内弧形小切口切开复位微型钛板内固定术治疗颧骨复合体骨折患者的效果

摘　　要：目的:观察发际内弧形小切口切开复位微型钛板内固定术治疗颧骨复合体骨折患者的效果。方法:选取2019年2月至2020年2月该院收治的88例颧骨复合体骨折患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各44例。观察组采取发际内弧形小切口切开复位微型钛板内固定术治疗,对照组予以头皮冠状切口切开复位微型螺钉内固定术治疗,比较两组手术相关指标(手术时间、切口长度、术中出血量)水平、术后康复指标(骨折愈合时间、住院时间)水平、疼痛程度[视觉模拟评分法(VAS)]评分和并发症发生率。结果:观察组手术时间、切口长度均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组骨折愈合时间、住院时间均短于对照组,差异有统计学意义(P<0.05);术后12、24、48 h,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05),术后72 h,两组VAS评分比较,差异无统计学意义(P>0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:发际内弧形小切口切开复位微型钛板内固定术治疗颧骨复合体骨折患者可降低手术相关指标水平、术后康复指标水平、VAS评分和并发症发生率,优于头皮冠状切口切开复位微型螺钉内固定术治疗效果。Objective:To observe effects of open reduction and mini titanium plate internal fixation through a small arc incision in the hairline on patients with zygomatic complex fractures.Methods:A prospective study was conducted on 88 patients with zygomatic complex fractures admitted to the hospital from February 2019 to February 2020.They were divided into control group and observation group according to the random number table method,44 cases in each.The observation group was treated with open reduction and mini titanium plate internal fixation through a small arc incision in the hairline,while the control group was treated with open reduction and micro screw internal fixation through a scalp coronal incision.The operation related index levels(operation time,incision length,intraoperative blood loss),the postoperative rehabilitation index levels(fracture healing time,hospitalization time),the pain[visual analogue scale(VAS)]score,and the complication rate were compared between the two groups.Results:The operation time and the incision length of the observation group were shorter than those of the control group;the intraoperative blood loss less than the control group;and the differences were statistically significant(P<0.05).The fracture healing time and the hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).12,24 and 48 h after the surgery,the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).72 h after the surgery,there was no significant difference in the VAS score between the two groups(P>0.05).The complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Open reduction and mini titanium plate internal fixation through a small arc incision in the hairline for the patients with zygomatic complex fractures can reduce the levels of operatio
