摘 要:糖尿病肾病是糖尿病微血管并发症之一,同时也是引起终末期肾脏病的重要病因。临床上主要以持续存在的白蛋白尿和(或)肾小球滤过率进行性下降为特征。糖尿病肾病致病机制复杂,目前普遍认为氧化应激、炎症和自噬是其主要的发病机制。临床上还没有针对糖尿病肾病治疗的药物,糖尿病肾病的治疗依然是亟待攻克的一个重点和难点。现对影响糖尿病肾病进展的三大主要发病机制进行一个总结性概括,以期为糖尿病肾病药物的研发带来一些新的思路和启发。Diabetic nephropathy is one of the microvascular complications of diabetes mellitus and an important cause of end-stage renal disease.It is characterized clinically by persistent albuminuria and/or a progressive decrease in glomerular filtration rate.The pathogenesis of diabetic nephropathy is complex,and oxidative stress,inflammation,and autophagy are generally considered to be its main pathogenic mechanisms.The treatment of diabetic nephropathy is still a key and difficult task to be tackled as there are no efficient drugs for the treatment of diabetic nephropathy.This article provides a summary of the three major pathogenic mechanisms affecting the progression of diabetic nephropathy,with the aim of bringing some new ideas and inspiration to the development of efficient drugs for the treatment of diabetic nephropathy.
