摘  要:目的观察通络糖泰方在波动性高血糖大鼠周围神经病变中的作用及机制。方法90只健康雄性SD大鼠随机分为正常对照组、稳定性高血糖组、波动性高血糖组、阳性对照组及通络糖泰方低、高剂量组,每组15只。5周后,测定大鼠热、冷刺激反应时间和坐骨神经传导速度;生化试剂测定大鼠空腹血糖(FBG)、糖化血红蛋白(GHb)含量,测定大鼠坐骨神经和红细胞的山梨醇含量,比色法测定大鼠超氧化物歧化酶(SOD)活性和丙二醛(MDA)含量。实时荧光定量PCR(RT-qPCR)检测坐骨神经组织NGF、Caspase-3 m RNA的表达。蛋白质印迹(Western Blot)检测坐骨神经组织Caspase-3和Nrf2通路相关蛋白(Nrf2、HO-1和NQO-1)表达。结果与正常对照组比较,稳定性高血糖组坐骨神经传导速度显著下降,血清FBG和GHb含量、坐骨神经组织山梨醇含量和红细胞山梨醇含量显著增加,血清SOD活性显著降低、MDA含量显著增加,坐骨神经组织NGF mRNA表达显著下调,Caspase-3 m RNA和蛋白及Nrf2、NQO-1、HO-1蛋白表达显著上调(P<0.05);波动性高血糖组上述指标变化比稳定性高血糖组更明显(P<0.05)。与波动性高血糖组比较,低、高剂量通络糖泰方显著降低热刺激反应时间,显著升高坐骨神经传导速度,降低血清FBG和GHb含量及坐骨神经组织山梨醇和红细胞山梨醇含量,显著升高血清SOD活性、降低MDA含量,显著上调坐骨神经组织NGF m RNA表达,显著下调Caspase-3 m RNA和蛋白及Nrf2、NQO-1、HO-1蛋白表达(P<0.05)。结论通络糖泰方可改变波动性高血糖大鼠周围神经病变情况,其机制可能与促进Nrf2通路蛋白表达有关。Objective To investigate the effect and mechanism of Tongluo Tangtai Decoction on peripheral neuropathy in rats with fluctuating hyperglycemia.Methods Ninety healthy male SD rats were randomly divided into normal control group,stable hyperglycemia group,fluctuating hyperglycemia group,positive control group,Tongluo Tangtai Decoction lowdose group and high-dose group,15 in each group.After 5 weeks,the reaction time to heat and cold stimulation and the conduction velocity of the sciatic nerve were measured;the content of fasting blood glucose(FBG)and glycosylated hemoglobin(GHb)were determined by biochemical reagents.Colorimetric determination of superoxide dismutase(SOD)activity and malondialdehyde(MDA)content in rats.Real-time fluorescent quantitative PCR(RT-qPCR)was used to detect the expression of NGF and Caspase-3 mRNA in sciatic nerve tissue.Western Blot was used to detect the expression of Caspase-3 and Nrf2 pathway related proteins(Nrf2,HO-1 and NQO-1)protein in sciatic nerve tissue.ResultssCompared with normal control group,the conduction velocity of the sciatic nerve the sciatic nerve conduction velocity in stable hyperglycemia group was significantly decreased,the content of FBG and GHb in serum,the content of sorbitol in sciatic nerve tissue and the content of sorbitol in erythrocyte were significantly increased,the activity of SOD was significantly decreased,the content of MDA was significantly increased,the expression of NGF mRNA in sciatic nerve tissue was significantly decreased,and Caspase-3 was significantly decreased MRNA and protein expression as well as Nrf2,NQO-1 and HO-1 protein expression were significantly up-regulated(P<0.05);the changes of above indexes in fluctuating hyperglycemia group were more obvious than those in stable hyperglycemia group.Compared with fluctuating hyperglycemia group,low and high dose Tongluo tangtai Decoction significantly reduced the response time to thermal stimulation,significantly increased the sciatic nerve conduction velocity,significantly increased serum FB
