“化湿和胃饮”联合盐酸阿扎司琼治疗恶性肿瘤化疗性恶心呕吐疗效研究

摘　　要：目的研究基于“脾阴理论”应用“化湿和胃饮”联合盐酸阿扎司琼治疗恶性肿瘤化疗性恶心呕吐(CINV)患者临床疗效。方法将我院肿瘤科CINV患者100例随机分为联合组(采用化湿和胃饮联合盐酸阿扎司琼治疗)与对照组(采用盐酸阿扎司琼治疗)各50例。比较两组恶心呕吐症状分级、恶心呕吐生活功能量表(FLIE)、Karnofsky功能状态(KPS)评分,血清5-羟色胺(5-HT)、多巴胺(DA)含量,不良反应发生率。结果治疗后,联合组恶心、呕吐症状严重程度低于对照组(P<0.05);两组患者FLIE评分明显降低,KPS评分明显升高,联合组FLIE评分明显低于对照组,KPS评分明显高于对照组(P<0.05);两组患者5-HT、DA含量明显降低,联合组5-HT、DA含量低于对照组(P<0.05);两组患者不良反应发生率对比,无统计学差异(P>0.05)。结论基于“脾阴理论”应用“化湿和胃饮”联合盐酸阿扎司琼对治疗恶性肿瘤化疗性恶心呕吐患者,能有效改善症状,降低血清5-HT、DA含量,改善功能状态,且并未增加不良反应。Objective To study the efficacy of Huashi Hewei Decoction combined with azasetron hydrochloride in treatment of chemotherapy induced nausea and vomiting(CINV)in patients with malignant tumors based on spleen yin theory.Methods One hundred patients with CINV were randomly divided into combination group and control group.Control group was treated with azasetron hydrochloride,and obsetvation group was treaded with Huashi Hewei Decction on the basis of control group.The grade of nausea and vomiting symptoms,the score of nausea and vomiting life functional living index emesis scale(FLIE),Karnofsky functional state(KPS),the content of serum 5-hydroxytryptamine(5-HT),dopamine(DA)and the incidence of adverse reactions were compared between the two groups.Results After treatment,the severity of nausea and vomiting in observation group was lower than control group(P<0.05);the FLIE score decreased significantly in both groups,and was significantly lower in observation group;the KPS score enhanced significantly,and was significantly higher in observation group(P<0.05);the contents of 5-HT and DA in both groups were significantly decreased,and were lower in observation group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Based on the spleen yin theory,Huashi Hewei Decoction combined with azasetron hydrochloride effectively improves symptoms,reduced serum 5-HT and DA levels,improved functional status,and doesn't increase adverse reactions in treatment of chemotherapy-induced nausea and vomiting in patients with malignant tumors.
