H型高血压患者血压变异性、MMP-9水平与大动脉弹性的相关性研究

摘　　要：目的探讨H型高血压患者血压变异性、血清基质金属蛋白酶-9(MMP-9)水平与大动脉弹性的关系。方法选取162例原发性高血压患者,依据血清同型半胱氨酸(Hcy)水平分为H型高血压患者(Hcy≥10μmol/L,86例)与非H型高血压患者(Hcy<10μmol/L,76例),选取健康对照组72例。监测患者24 h动态血压(ABMP),计算血压变异指标(BPV),测定患者血清Hcy、MMP-9水平和肱动脉-踝动脉脉搏波传导速度(baPWV)、比较得到各组差异及关系。结果H型高血压组24 h平均收缩压(24 h-SBP)、白昼平均收缩压(dSBP)、夜间平均收缩压(nSBP)、24 h收缩压标准差(24 h-SBPSD)、白昼收缩压标准差(dSBPSD)、白昼收缩压变异系数(dSBPCV)、夜间收缩压标准差(nSBPSD)、夜间收缩压变异系数(nSBPCV)、Hcy水平高于非H型高血压组(P<0.05)。H型高血压组MMP-9、baPWV指标明显高于非H型高血压组(P<0.05)。两变量直线相关分析显示H型高血压组Hcy与baPWV正相关(r=0.326,P<0.05),MMP-9与baPWV、Hcy正相关(r=0.547、0.289,P<0.05)。多元线性回归分析显示24 h-SBPCV、24 h-SBPSD、dSBPSD、nSBPSD、MMP-9、Hcy、LDL-C为baPWV主要影响因素。结论H型高血压患者血压变异性、MMP-9水平与大动脉弹性有密切关系。Objective To investigate the correlation of blood pressure variability,matrix metallopoeinases-9(MMP-9)level and arterial elasticity in patients with H-type hypertension.Methods 162 patients with essential hypertension were divided into the hyperhomocysteinemia group(H-type hypertension,Hcy≥10μmol/L,n=86)and non-H-type hypertension group(Hcy<10μmol/L,n=76)according to the plasma hyperhomocysteinemia(Hcy),and the healthy people with normal blood pressure were served as the control group(n=72).The 24-hour ambulatory blood pressure(ABMP),blood pressure variability index(BPV),serum Hcy,MMP-9 levels and brachial-ankle pulsewave velocity(baPWV)were measured and analyzed.Results 24h-SBP,dSBP,nSBP,24h-SBPSD,24h-SBPCV,dSBPSD,dSBPCV,nSBPSD,nSBPCV,Hcy,MMP-9 and baPWV in H-type hypertension group were significantly higher than those in non-H-type hypertension group(P<0.05).Two variables linear correlation analysis showed that the Hcy was positively correlated with baPWV in H-type hypertension(r=0.326,P<0.05)and the MMP-9 was positively correlated with baPWV and Hcy(r=0.547、0.289,P<0.05).Multiple linear regression analysis showed that 24h-SBPCV,24h-SBPSD,dSBPSD,nSBPSD,MMP-9,Hcy and LDL-C were the main effect factors of baPWV.Conclusion BPV and MMP-9 are significantly correlated with arterial elasticity in patients with H-type hypertension.
