摘 要:近年来微生物组学的发展为皮肤病学的研究提供了新的方法,皮肤和肠道微生态的失调参与了多种皮肤炎症性疾病的调控。头皮作为独特的微生物物种相关的亚栖息地,其微生态的研究也为毛发疾病病理机制的研究打开了新视角。本文对脂溢性皮炎、斑秃、雄激素性脱发、头皮银屑病等常见头皮毛发疾病的微生态改变及其相关新兴治疗药物包括Janus激酶(Janus kinase,JAK)-转录激活因子(transcriptional activator,STAT)抑制剂等小分子药物和度普利尤单抗(dupilumab)、司库奇尤单抗(secukinumab)、乌司奴单抗(ustekinumab)等生物制剂对微生态的影响进行综述,阐明皮肤-肠道微生态在维持头皮正常状态和常见毛发疾病病理机制中的重要作用,从而为这些疾病的预防、临床诊疗和疗效评估等提供新的方向。In recent years,the development of microbiomics has provided a novel method for dermatological research.Dysbiosis of skin or gut microbiome is reported to be involved in the regulation of various inflammatory skin diseases.As a unique sub-habitat associated with microbial species,the study of scalp microbiome also explored a new perspective for pathological mechanisms of hair diseases.In this paper,we reviewed microbiological changes of several common scalp and hair diseases,including seborrheic dermatitis(SD),alopecia areata(AA),androgenetic alopecia(AGA)and scalp psoriasis.The effects of small molecule drugs including Janus kinase(JAK)-Signal transducer and activator of transcription(STAT)inhibitors and biological agents such as dupilumab,secukinumab and ustekinumab on microbiome are also discussed.It will be useful to clarify important roles of skin and gut microbiome in the maintenance of scalp homeostasis,and the pathological mechanisms of common hair diseases,thus providing new directions for the prevention,clinical treatment and efficacy evaluation of these diseases.
