张氏益胃汤联合质子泵抑制剂治疗脾胃气虚型幽门螺旋杆菌阳性消化性溃疡随机对照试验

摘　　要：目的:评价张氏益胃汤联合质子泵抑制剂治疗脾胃气虚型幽门螺旋杆菌(Hp)阳性消化性溃疡(PU)患者疗效。方法:采用随机对照试验方法,纳入脾胃气虚型Hp阳性PU患者64例,随机分为观察组(张氏益胃汤联合质子泵抑制剂)和对照组(四联疗法)。观察2组患者胃镜疗效、Hp根除率、胃肠激素水平、症状评分以及安全性。结果:观察组胃镜疗效总显效率为68.75%,总有效为87.50%;对照组胃镜疗效总显效率为62.50%,总有效率为78.13%,2组差异有统计学意义(P<0.05)。观察组与对照组Hp根除率总有效率分别为87.50%、81.25%,2组差异有统计学意义(P<0.05)。2组血清胃泌素(GAS)治疗前后差值比较,观察组降低患者GAS水平优于对照组(P<0.05)。2组血清生长抑素(SS)治疗前后差值比较,观察组升高患者SS水平优于对照组(P<0.05)。除神疲懒言症状(2组没有差异)外,其他症状评分观察组均优于对照组。2组试验期间均未出现不良事件,没有病例由于不良反应退出。结论:张氏益胃汤联合质子泵抑制剂治疗Hp相关性PU可提高治疗疗效、改善患者不适症状。Objective:To evaluate the efficacy of Zhangshi Yiwei decoction combined with proton pump inhibitor in the treatment of Helicobacter pylori(HP)Positive Peptic Ulcer(PU)with spleen and stomach qi deficiency.Methods:64 patients with HP positive PU of spleen stomach qi deficiency type were randomly divided into treatment group(Zhang Shi Yiwei decoction combined with proton pump inhibitor)and control group(quadruple therapy).The curative effect of gastroscopy,HP eradication rate,gastrointestinal hormone level,symptom score and safety of the two groups were observed.Results:The total effective rate of gastroscopy in the treatment group was 68.75%,and the total effective rate was 87.50%;The total effective C rate of gastroscopy in the control group was 62.50%,and the total effective rate was 78.13%.There was significant difference between the two groups(P<0.05).The total effective rates of HP eradication in the treatment group and the control group were 87.50%and 81.25%respectively,and there was significant difference between the two groups(P<0.05).Compared with the difference before and after treatment,the gas level in the treatment group was lower than that in the control group(P<0.05).The difference of serum somatostatin(SS)between the two groups before and after treatment showed that the level of ss in the treatment group was higher than that in the control group(P<0.05).Except for the symptoms of mental fatigue and lazy speech(there is no difference between the two groups),the treatment group of other symptoms is better than the control group.There are no adverse events in the two groups during the trial,and no cases withdraw due to adverse reactions.Conclusion:Zhangshi Yiwei decoction combined with proton pump inhibitor in the treatment of HP related Pu can improve the therapeutic effect and improve the discomfort symptoms of patients.
