内热针治疗早中期膝骨关节炎的疗效观察及对血清IL-1β、YKL-40水平的影响

摘　　要：目的 观察内热针治疗早中期膝骨关节炎患者的临床疗效及其对血清白介素-1β(interleukin-1β,IL-1β)、几丁质酶3样蛋白1(chitinase-3-like protein 1, YKL-40)水平的影响。方法 将180例早中期膝骨关节炎患者随机分为观察组与对照组,每组90例。观察组运用内热针治疗,对照组运用玻璃酸钠腔内注射联合口服塞来昔布胶囊。比较两组治疗前、治疗后、治疗后3个月和治疗后6个月视觉模拟量表(visual analog scale,VAS)评分、西安大略麦马斯特大学骨关节炎指数(Western Ontario and McMaster Universities osteoarthritis index, WOMAC)评分及血清IL-1β、YKL-40水平变化。结果 两组治疗后、治疗后3个月和治疗后6个月VAS评分、WOMAC评分及血清IL-1β、YKL-40水平均低于治疗前(P<0.05),观察组低于对照组(P<0.05)。结论 内热针治疗早中期膝骨关节炎患者效果优于玻璃酸钠腔内注射联合口服塞来昔布,能够缓解关节疼痛,改善关节功能,提高患者的生存质量,并且使血清IL-1β、YKL-40水平下降。Objective To observe the clinical efficacy of inner heating dry needle therapy in the treatment of patients with early to mid-stage knee osteoarthritis and its effect on serum interleukin-1β(IL-1β)and chitinase-3-like protein 1(YKL-40)levels.Method A total of 180 patients with early to mid-stage knee osteoarthritis were randomly divided into an observation group and a control group,with 90 cases in each group.The observation group was treated with inner heating dry needle,and the control group was treated with sodium hyaluronate intra-cavity injection combined with oral celecoxib capsules.The changes in visual analog scale(VAS)score,Western Ontario and McMaster Universities osteoarthritis index(WOMAC)knee score,and serum IL-1β and YKL-40 levels were compared between the two groups before treatment,after treatment,3 months after treatment and 6 months after treatment.Result The VAS score,WOMAC knee score and serum IL-1β and YKL-40 levels after treatment,3 months and 6 months after treatment were lower than those before treatment in the two groups(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).Conclusion The effect of inner heating dry needle therapy is better than sodium hyaluronate intra-cavity injection combined with oral celecoxib in the treatment of patients with early to mid-stage knee osteoarthritis.It can relieve joint pain,improve joint function,improve the quality of life of patients,and reduce the levels of serum IL-1β and YKL-40.
