别嘌醇致HLA-B^(*)58:01阴性的慢性肾病患者大疱性表皮松解坏死型药疹

摘　　要：报告别嘌醇致HLA-B^(*)58:01阴性的慢性肾病患者大疱性表皮松解坏死型药疹1例。患者女性,慢性肾脏病合并高尿酸血症。行HLA-B^(*)58:01基因检测,结果阴性。使用别嘌醇治疗,出现大疱性表皮松解坏死型药疹,予以停用别嘌醇以及血浆置换联合连续血液透析滤过、糖皮质激素抗炎治疗,患者病情好转。A case of bullous epidermolytic necrotic drug eruption induced by allopurinol in HLA-B^(*)58:01 allele-negative patient with chronic kidney disease is reported.A female patient with chronic kidney disease and hyperuricemia developed bullous epidermolytic necrotic drug eruption after allopurinol treatment.The patient was tested negative for HLA-B ~*58:01 gene.The condition was improved after discontinuation of allopurinol,plasma exchange combined with continuous hemodialysis and corticosteroid anti-inflammatory treatment.
