摘  要:目的探讨黄芪甲苷(ASⅣ)联合高压氧(HBO)处理对心肌缺血再灌注损伤模型大鼠心肌梗死范围及心肌细胞凋亡的影响。方法SPF级SD雄性大鼠40只,按照随机数字表法分为4组:对照组、模型组、ASⅣ组及ASⅣ+HBO组,每组10只。对照组不作任何处理;模型组通过结扎左冠状动脉前降支构建心肌缺血再灌注损伤大鼠模型;ASⅣ组为模型大鼠冠状动脉结扎后经十二指肠注射羧甲基纤维素钠,冠状动脉解除结扎后即刻十二指肠注射3 g/kg ASⅣ;ASⅣ+HBO组冠状动脉解除结扎后于十二指肠注射3 g/kg ASⅣ,并同时开始给予0.25 MPa(2.5 ATA)的HBO处理。测量并计算各组大鼠心肌缺血梗死范围,酶联免疫吸附实验检测各组大鼠血清中细胞色素C(Cyt-C)含量,TURNEL实验检测各组大鼠心肌细胞的凋亡情况,免疫组化(SP)法检测各组大鼠心肌细胞中凋亡蛋白的表达量。结果与ASⅣ组比较,ASⅣ+HBO组大鼠心肌梗死面积和梗死面积占比均明显缩小或降低(P<0.05);血清Cyt-C水平明显降低(P<0.05);心肌凋亡细胞数量最少,细胞凋亡率明显降低(P<0.05);ASⅣ+HBO组心肌细胞内凋亡蛋白Bcl-2表达明显上调,Bax、caspase-3蛋白表达明显下调(P<0.05)。结论ASⅣ联合HBO处理能够有效地降低心肌缺血再灌注损伤模型大鼠心肌梗死的范围,其主要作用机制是通过调控心肌细胞凋亡蛋白的表达水平,从而相应减少缺血再灌注损伤后心肌细胞凋亡的数量来实现的。Objective To investigate the effects of hyperbaric oxygen(HBO)combined with astragalosideⅣ(ASⅣ)on myocardial infarction area and myocardial cell apoptosis in rat model of myocardial ischemia-reperfusion injury.Methods A total of 40 SPF-grade SD rats were divided into normal control group,model control group,ASⅣgroup,and ASⅣ+HBO group,with 10 rats in each group.The myocardial ischemia-reperfusion rat model was constructed by ligating the left anterior descending coronary artery;the rat models in the ASⅣgroup received duodenal injection of sodium carboxymethylcellulose after coronary artery ligation,and ASⅣinjection at a concentration of 3 g/kg into the duodenum immediately after coronary artery ligation;the rat models received injection of ASⅣat a concentration of 3 g/kg into the duodenum immediately after coronary artery ligation ASⅣand were given 0.25 MPa(2.5 ATA)HBO at the same time.After all groups received interventions,the myocardial infarction area was measured,the content of Cyt-C in serum was determined by ELISA,and myocardial cell apoptosis were tested by TURNEL,the expressions of apoptosis proteins in rat myocardial cell were measured by immunohistochemical staining(SP method).Results Compared with the ASⅣgroup,in the ASⅣ+HBO group:the myocardial infarction area and the area ratio of myocardial infarction were all reduced(P<0.05);serum Cyt-C content was increased significantly(P<0.05);the count of apoptosis cell was the lowest,and cell apoptosis rate obviously decreased(P<0.05);the expression of Bcl-2 apoptosis protein in myocardial tissue was up-regulated,while the expressions of Bax and Caspase-3 proteins were down-regulated(P<0.05).Conclusion ASⅣcombined with HBO can effectively reduce the myocardial infarction area in rat model of myocardial ischemia-reperfusion injury.The main mechanism of action is regulating the expressions of myocardial cell apoptosis proteins,which,therefore,reduces the count of apoptosis cell after myocardial ischemia-reperfusion injury.
