术后植入全降解鼻窦药物支架在嗜酸性粒细胞型慢性鼻窦炎患者应用价值的探讨

摘　　要：目的探讨嗜酸性粒细胞型鼻息肉患者术后2周植入鼻窦药物支架的短期疗效和安全性。方法收集广州医科大学附属第二医院耳鼻咽喉科36例确诊患者,均为双侧发病。将其随机分为2组,每组18例,全麻下鼻内镜下行全组鼻窦开放手术,术中植入组在术中同期植入鼻窦药物支架,植入组在术后2周表面麻醉下清理术腔后植入鼻窦药物支架。术后随访3个月,术腔进行症状及黏膜评估,采用视觉模拟量表、Lund-Kennedy评分法。结果术后植入组和术中植入组患者鼻部的各症状评分在术后4、8、12周均有统计学差异,且在术后8、12周时鼻内镜评分(息肉、水肿、鼻漏)对比统计均显著降低,均有统计学差异(P<0.05)。结论嗜酸性粒细胞型鼻息肉患者在术后2周植入鼻窦药物支架有较好的安全性及有效性。OBJECTIVE To investigate the short-term efficacy and safety of implanting drug stents into nasal sinuses in patients with eosinophilic nasal polyps 2 weeks after surgery.METHODS Thirty-six patients diagnosed in Department of Otolaryngology Head and Neck Surgery,the Second Affiliated Hospital of Guangzhou Medical University,were all bilateral.They were randomly divided into two groups,18 cases in each group.Under general anesthesia,both the groups underwent endoscopic sinus surgery.The intraoperative implantation group implanted drug stents in the sinuses during the surgery at the same time.After the surgery,the postoperative implantation group implanted drug stents in the sinuses after cleaning the surgical cavity under surface anesthesia two weeks after the surgery.The patients were followed up for 3 months,and the symptoms and mucous membrane were evaluated in the operation cavity,using visual analog scale and Lund Kennedy scoring method.RESULTS The scores of nasal symptoms in the postoperative implantation group and the intraoperative implantation group were statistically different at 4,8 and 12 weeks after surgery,and the scores of nasal endoscopy(polyps,edema,rhinorrhea)were significantly reduced at 8 and 12 weeks after surgery(P<0.05).CONCLUSION It is safe and effective for patients with eosinophilic nasal polyps to implant drug stents into nasal sinuses 2 weeks after surgery.
