直视人流系统与超声引导下人流术对患者子宫内膜恢复的影响及宫腔黏连发生风险比较

摘　　要：目的探讨直视人流系统与超声引导下人流手术的效果、并发症发生情况。方法选取我院2021年12月至2022年11月因意外妊娠或病理妊娠要求终止妊娠且自愿选择静脉麻醉下人工流产手术的早期妊娠(妊娠≤10周)患者100例,采用随机数字表法分为两组,其中直视组50例采用直视人流系统下人流手术治疗,超声组50例采用超声引导下人流手术治疗,比较两组患者的围手术期指标、宫腔粘连发生情况。结果直视组患者的术中出血量低于超声组,手术时间、腹痛持续时间、月经恢复时间均短于超声组(P<0.05);两组阴道出血时间比较差异无统计学意义(P>0.05);两组术后2、3、4周尿HCG转阴率比较,差异均无统计学意义(P>0.05);两组术后14、28天子宫内膜厚度比较差异无统计学意义(P>0.05);随访3个月,直视组子宫宫腔粘连程度低于超声组(P<0.05)。结论直视人流系统与超声引导下人流手术的效果相当,但直视人流系统具有手术时间短、手术出血量少、术后月经恢复快,宫腔粘连程度轻的优势。Objective To explore the effects and complications of direct vision abortion system and ultrasound-guided abortion surgery.Methods From December 2021 to November 2022,100 patients with early pregnancy(pregnancy≤10 weeks)who required to terminate pregnancy due to unexpected pregnancy or pathological pregnancy or voluntarily chose artificial abortion under intravenous anesthesia in our hospital were selected.The patients were divided into a direct vision group and an ultrasonic group,50 in each group by random number table method.The direct vision group was treated with artificial abortion under the direct vision system,and the ultrasonic group was treated with artificial abortion under the guidance of ultrasound.The perioperative indexes and uterine cavity adhesion were compared between the two groups.Results The intraoperative bleeding volume was lower and the operation time,duration of abdominal pain and menstrual recovery time were shorter or lower in the direct vision group than those in the ultrasound group(P<0.05).There was no significant difference in the time of vaginal bleeding between the two groups(P>0.05).There was no significant difference in the rate of urine HCG negative conversion between the two groups after 2,3 and 4 weeks of operation(P>0.05).There was no significant difference in endometrial thickness between the two groups after 14 and 28 days of operation(P>0.05).After patients followed-up for 3 months,the degree of uterine cavity adhesion in the direct vision group was lower than that in the ultrasound group(P<0.05).Conclusions The effect of direct vision artificial abortion system is similar to that of ultrasound-guided artificial abortion operation.However,the direct vision artificial abortion system has advantages of short operation time,less operation bleeding,fast postoperative menstrual recovery,and light uterine cavity adhesion.
