健康中国背景下基层医疗卫生服务对居民幸福感的影响研究

摘　　要：目的:分析基层医疗卫生服务对居民幸福感的影响,并验证影响的程度是否会因居民个体社会公平感的不同而存在异质性,对“强基层”改革进行评价并提供决策依据。方法:采用2017中国综合社会调查及相关统计年鉴数据,构建分层模型考察基层医疗卫生服务能力对居民幸福感的直接作用,并验证居民社会公平感与基层医疗卫生服务能力的交互作用对幸福感的影响。结果:直接作用检验结果显示,每万人口全科医生数(0.039,P<0.01)、每千人口专业公共卫生机构人员数(0.041,P<0.1)的回归系数显著为正;交互作用检验结果显示,居民社会公平感与每万人口全科医生数的交互项回归系数显著为负(-0.007,P<0.05)。结论:专业公共卫生机构人员数量对居民幸福感的正向影响具有同质性;全科医生数量对居民幸福感的正向影响具有异质性,影响程度会随着居民社会公平感的降低而增强。建议把加强人才队伍建设作为“强基层”改革的重点,此外,也要加强基层机构的心理健康服务能力。Objective To analyze the influence of primary medical and health services on residents’subjective well-being(SWB),and evaluate and provide policy suggestions for the reform of strengthening primary medical and health services.Methods Based on CGSS 2017 and relevant provincial data,multilayer linear model(HLM)was carried out to analyze the direct effects of primary medical and health services on SWB,and verify the interaction between social fairness perception(SFP)and the capability of primary medical and health services and its influence on residents’SWB.Results The examination of direct effects showed that the regression coefficient of the number of GPs per 10000 population(0.039,P<0.01)and the number of public health professionals per 1000 population(0.041,P<0.1)was significantly positive.The examination of interaction showed that the regression coefficient of interaction term between the number of GPs per 10000 population and SFP was significantly negative(-0.007,P<0.05).Conclusion The positive impact of the number of public health professionals on residents’SWB was homogeneous.The positive impact of the number of GPs on residents’SWB was heterogeneous.The lower the residents’SFP,the stronger the positive impact on their SWB.It is suggested to take the construction of talent team as the focus of reform on strengthening primary medical and health services,and improve the mental health service ability of primary institutions.
