摘　　要：目的分析分级诊疗制度下孕妇首诊选择的影响因素。方法选取杭州市临安区2017-2021年就诊的14253例孕妇,采用问卷调查孕妇首诊选择情况及影响因素。结果孕妇首诊选择时最看重医疗技术水平。单因素分析显示,孕妇首诊选择二级医疗机构中年龄≤35岁、大学本科及以上、新型农村合作医疗保险、未合并基础疾病、了解分级诊疗情况的占比大于三级医疗机构(p<0.05)。多因素分析显示,年龄、受教育程度、医保类别、是否合并基础疾病、分级诊疗知晓情况为影响孕妇首诊选择的因素(OR=5.068、4.491、4.446、5.028、3.543,p<0.05)。结论孕妇首诊选择时最看重医疗技术水平,年龄、受教育程度、医保类别、是否合并基础疾病、分级诊疗知晓情况对孕妇首诊选择具有显著影响。Objective To analyze the influencing factors of pregnant women's choice of first visit under the graded diagnosis and treatment system.Methods A total of 14,253 pregnant women from 2017 to 2021 in Lin'an District,Hangzhou were selected,and a questionnaire was used to investigate the choice of first visit and its influencing factors.Results The level of medical technology was the most important for pregnant women to choose their first visit.Univariate analysis showed that pregnant women under the age of 35,with a bachelor's degree or above,new rural cooperative medical insurance,no basic diseases,and knowledge of graded diagnosis and treatment were more willing to choose secondary medical institutions than tertiary medical institutions.(p<0.05)Multivariate analysis showed that age,education level,medical insurance category,whether or not having the basic diseases,and the knowledge of graded diagnosis and treatment were the factors influencing the choice of first visit for pregnant women(OR=5.068,4.491,4.446,5.028,3.543,p<0.05).Conclusion The level of medical technology is the most important choice for pregnant women's first visit.Age,education level,medical insurance category,whether or not having basic diseases and the knowledge of graded diagnosis and treatment have significant influence on the choice of first visit for pregnant women.
