摘　　要：目的:探讨不同缩宫素用法对剖宫产产后出血产妇的影响。方法:将2019年3月—2022年3月90例在该院进行剖宫产的产妇作为观察对象,按照产妇住院时间的单双号分为45例对照组和45例治疗组,两组产妇在分娩出胎儿后子宫肌层注射10 U的缩宫素,然后治疗组静脉滴注20 U的缩宫素,对照组在治疗组基础上静脉推注5 U的缩宫素,比较两组血流动力学、第三产程时间、术后出血情况及不良反应发生情况。结果:治疗组用药后30 s~5 min收缩压高于对照组,心率低于对照组(P<0.05);治疗组用药1 min后舒张压高于对照组(P<0.05);治疗组第三产程时间以及术后2 h、24 h出血量低于对照组(P<0.05);治疗组不良反应发生率低于对照组(P<0.05)。结论:分娩出胎儿后子宫肌层注射10 U的缩宫素,然后再静脉滴注20 U的缩宫素可改善产妇的血流动力学、缩短第三产程时间、减少出血量,且安全性较好。Objective:To investigate the effect of different oxytocin usage on postpartum hemorrhage in cesarean section.Methods:From March 2019 to March 2022,90 parturients who underwent cesarean section in our hospital were selected as the observation objects.According to the odd and even number of maternal hospitalization time,they were divided into 45 cases of control group and 45 cases of treatment group.The two groups of parturients were injected with 10 U oxytocin into the myometrium after delivery of the fetus,and then the treatment group was intravenously injected with 20 U oxytocin.The control group was intravenously injected with 5 U oxytocin on the basis of the treatment group.The hemodynamics,the third stage of labor,postoperative bleeding and adverse reactions were compared between the two groups.Results:The systolic blood pressure of 30 s-5 min in the treatment group was higher than that in the control group,and the heart rate was lower than that in the control group(P<0.05).The diastolic blood pressure in the treatment group was higher than that in the control group after 1 min(P<0.05).The duration of the third stage of labor and the blood loss at 2 and 24 h after operation in the treatment group were lower than those in the control group(P<0.05).The incidence of adverse reactions in treatment group was lower than that in control group(P<0.05).Conclusion:Injection of 10 U oxytocin into myometrium after delivery and then intravenous infusion of 2o U oxytocin can improve maternal hemodynamics,shorten the third stage of labor,reduce the amount of bleeding,and have good safety.
